Precautions and warnings
न्यूरोमस्कुलर ब्लॉकिंग एजेंटों के साथ-साथ या मायस्थेनिया ग्रेविस, पार्किंसंस रोग वाले लोगों या कान और गुर्दे की क्षति के जोखिम वाले लोगों में जेंटामाइसिन का उपयोग करते समय सतर्क रहें। स्तनपान के दौरान परहेज करें। उच्च खुराक या लंबे समय तक उपचार लेने वाले रोगियों के रक्त में जेंटामाइसिन के स्तर की जाँच करें - विशेष रूप से शिशुओं, बुजुर्ग लोगों, गुर्दे की समस्याओं वाले, सिस्टिक फाइब्रोसिस या महत्वपूर्ण मोटापे से ग्रस्त लोगों के लिए। उपचार के दौरान नियमित रूप से सुनने और गुर्दे की कार्यप्रणाली की निगरानी करें।
Side-effects

Drug Interactions
**जेंटामाइसिन:**
जेंटामाइसिन को अन्य दवाओं के साथ लेना जो तंत्रिकाओं या गुर्दे को नुकसान पहुंचा सकती हैं - जैसे कि सेफलोस्पोरिन, मेथिसिलिन, एम्फोटेरिसिन बी, सिक्लोस्पोरिन, सिस्प्लैटिन, मजबूत मूत्रवर्धक (जैसे एथैक्रिनिक एसिड या फ़्यूरोसेमाइड), या मांसपेशियों को आराम देने वाले (जैसे, स्यूसिनिलकोलाइन, ट्यूबोक्यूरिन) - साइड इफेक्ट का खतरा बढ़ सकता है। जेंटामाइसिन रक्त को पतला करने वाली दवाओं (जैसे, वारफारिन, फेनिंडियोन) को अधिक मजबूती से काम कर सकता है। यह नियोस्टिग्माइन और पाइरिडोस्टिग्माइन जैसी दवाओं के प्रभाव को कम कर सकता है। यदि बिसफ़ॉस्फ़ोनेट्स के साथ लिया जाए तो निम्न रक्त कैल्शियम का जोखिम अधिक होता है, और बोटुलिनम विष के साथ लेने पर मांसपेशियों में कमजोरी की संभावना बढ़ जाती है। नवजात शिशुओं में, इंडोमेटासिन रक्त में जेंटामाइसिन के उच्च स्तर का कारण बन सकता है।
**हाइड्रोकार्टिसोन:**
थियाजाइड मूत्रवर्धक हाइड्रोकार्टिसोन के कारण होने वाले उच्च रक्त शर्करा और निम्न रक्त पोटेशियम को खराब कर सकता है। एक ही समय में गैर-स्टेरायडल विरोधी भड़काऊ दवाएं (एनएसएआईडी) लेने से पेट में अल्सर या रक्तस्राव का खतरा बढ़ जाता है। हाइड्रोकार्टिसोन एंटीकोआगुलंट्स के काम करने के तरीके को बदल सकता है, और लोगों को मधुमेह या रक्तचाप की दवाओं की खुराक बढ़ाने की आवश्यकता हो सकती है। यह रक्त में सैलिसिलेट और एंटीमस्करिनिक दवाओं के स्तर को कम करता है, और शराब पीने से पेट की जलन बदतर हो सकती है। कार्बामाज़ेपाइन, फ़िनाइटोइन, प्राइमिडोन, बार्बिट्यूरेट्स और रिफैम्पिसिन जैसी दवाएं हाइड्रोकार्टिसोन के काम करने की क्षमता को कम कर सकती हैं। जब सिक्लोस्पोरिन के साथ लिया जाता है, तो दोनों दवाएं रक्त में एक-दूसरे के स्तर को बढ़ा देती हैं। एस्ट्रोजेन या जन्म नियंत्रण की गोलियाँ लेने वाली महिलाओं में हाइड्रोकार्टिसोन अधिक मजबूती से काम कर सकता है।
Contraindications
यदि आपके पास अमीनोग्लाइकोसाइड्स से एलर्जी प्रतिक्रियाओं का इतिहास है, गर्भवती हैं, या यकृत की समस्याएं हैं तो इसका उपयोग न करें।
Mode of actions
जेंटामाइसिन एक एमिनोग्लाइकोसाइड एंटीबायोटिक है जो बैक्टीरिया में राइबोसोम के 30 और 50 के हिस्से से जुड़कर काम करता है। यह बैक्टीरिया को उनके लिए आवश्यक प्रोटीन बनाने से रोकता है, जो उनकी कोशिका झिल्ली को नुकसान पहुंचाता है और अंततः उन्हें मार देता है।
हाइड्रोकार्टिसोन एक कॉर्टिकोस्टेरॉइड दवा है जिसका उपयोग इसलिए किया जाता है क्योंकि यह सूजन को कम करती है और प्रतिरक्षा प्रणाली को दबा देती है। यह कुछ श्वेत रक्त कोशिकाओं को सूजन वाले क्षेत्रों में जाने से रोककर और रक्त वाहिकाओं को कम रिसावयुक्त बनाकर ऐसा करता है। हाइड्रोकार्टिसोन का उपयोग उन लोगों में हार्मोन को बदलने के लिए भी किया जा सकता है जो अपने आप पर्याप्त हार्मोन नहीं बनाते हैं, जैसे कि एड्रेनोकोर्टिकल अपर्याप्तता वाले लोग।
Dosage & Administration
ऑप्थ सतही ऑप्थ संक्रमण 0.3% सोलन के रूप में: 1-2 बूंदें 4 घंटे बाद डालें, गंभीर मामलों में 2 बूंदें हर घंटे डालें।
Pregnancy & Lactation
यद्यपि पशु प्रजनन अध्ययनों ने भ्रूण पर प्रतिकूल प्रभाव दिखाया है और गर्भवती महिलाओं में कोई पर्याप्त और अच्छी तरह से नियंत्रित अध्ययन नहीं हैं, लेकिन गर्भावस्था के दौरान, संभावित जोखिमों के बावजूद, इस दवा का उपयोग केवल तभी किया जा सकता है जब इसकी आवश्यकता हो।
जिन माताओं के नवजात शिशु हैं, जिन्होंने गर्भावस्था के आखिरी 3 महीनों के दौरान इस दवा का उपयोग किया है, उनमें शायद ही कभी उनींदापन, मांसपेशियों में अकड़न या कंपकंपी, दूध पीने या सांस लेने में परेशानी, लगातार रोना जैसे लक्षण पैदा हो सकते हैं। यदि आपको विशेष रूप से उनके पहले महीने के दौरान अपने बच्चे में इनमें से कोई भी लक्षण दिखाई देता है, तो तुरंत डॉक्टर को बताएं।
यह दवा स्तन के दूध में पारित हो जाती है। स्तनपान कराने से पहले अपने डॉक्टर से बात करें।
