Precautions and warnings

Kidney problems; congestive heart failure (CHF) or a history of CHF; chronic lung disease (COPD); monitor liver function.

Drug Interactions

This medication can raise the levels of certain drugs in your blood, including oral anticoagulants, digoxin, cilostazol, alprazolam, IV midazolam, repaglinide, and some corticosteroids like budesonide, dexamethasone, fluticasone, and methylprednisolone. It can also increase the levels of HIV protease inhibitors (such as ritonavir, indinavir, saquinavir), erythromycin, and clarithromycin.

On the other hand, it may lower the levels of drugs like isoniazid, carbamazepine, nevirapine, phenytoin, phenobarbital, rifampicin, and rifabutin. It can reduce drug absorption when taken with proton pump inhibitors (PPIs), antacids, antimuscarinics, or H2 blockers. Using it with dihydropyridines may cause swelling (edema). It can also worsen the heart effects of verapamil and increase the risk of severe breathing problems when taken with fentanyl.

**Serious Risks:**
This medication may increase the risk of dangerous heart rhythm problems (QT prolongation or torsades de pointes) if used with certain drugs like astemizole, bepridil, cisapride, dofetilide, levometadyl, mizolastine, pimozide, quinidine, sertindole, terfenadine, methadone, ranolazine, dronedarone, and halofantrine.

It can also raise the risk of muscle problems (myopathy or rhabdomyolysis) if taken with cholesterol-lowering drugs like atorvastatin, lovastatin, or simvastatin, and can raise the risk of ergotism if used with ergot alkaloids like ergotamine.

Finally, it may increase the sedative effects of triazolam and oral midazolam, and boost the blood levels of drugs like eletriptan, nisoldipine, felodipine, disopyramide, irinotecan, lurasidone, and colchicine (especially in patients with kidney or liver problems). There is also a risk of low blood pressure and high potassium with eplerenone.

Contraindications

Allergic reactions to azole antifungal medications, pregnancy, and breastfeeding are reasons you should not use this drug. It should also be avoided in people with liver disease. For intravenous (IV) use, kidney function (CrCl) needs to be considered.

Mode of actions

Itraconazole works by blocking cytochrome P450, which lowers the amount of ergosterol made by fungi. Without enough ergosterol, the fungi can’t maintain their cell membranes, so they become weaker and can’t survive. This makes itraconazole effective against a wide range of fungi, including Microsporum, Trichophyton, Candida, Aspergillus, Epidermophyton, Cryptococcus, Histoplasma, Blastomyces, Sporothrix, Malassezia, Coccidioides, and Paracoccidioides. In addition, itraconazole can also help fight certain protozoan parasites like Leishmania.

Dosage & Administration

Adult: PO: Oropharyngeal candidiasis As cap: 100 mg/day for 15 days.
Vulvovaginal candidiasis As cap: 200 mg twice daily for 1 day.
Pityriasis versicolor As cap: 200 mg/day for 7 days.
Tinea corporis; Tinea cruris As cap: 100 mg/day for 15 days.
Fungal nail infections As cap: 200 mg/day for 3 mth.
Systemic fungal infections As cap: 100-200 mg once daily, up to 200 mg twice daily for invasive or disseminated infections.
Prophylaxis of infections in neutropenic or AIDS patients As cap: 200 mg/day, up to 200 mg twice daily if needed.
Tinea pedis; Tinea manuum As cap: 100 mg/day for 30 days.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
