Precautions and warnings

Tell your doctor or pharmacist about your medical history before using this drug, especially if you have liver disease, renal illness, or certain blood problems such as low blood cell counts.

Metronidazole use may put people with a rare genetic condition at risk for extremely serious liver damage. Consult your doctor about the dangers and advantages. Your doctor will assess your liver function if metronidazole is prescribed. If you experience any symptoms of liver disease during treatment, such as uncontrollable vomiting, lack of appetite, severe stomach or abdominal discomfort, yellowing eyes, or dark urine, get medical attention straight once.

While taking this drug and for at least 6 days after stopping it, avoid alcoholic beverages and items containing propylene glycol since severe stomach upset, nausea, vomiting, headache, and flushing may occur.

This medication has the potential to make you dizzy. You may feel more dizzy than usual if you consume alcohol or use cannabis. Do not drive, operate machinery, or engage in any other activity that requires attentiveness until you are sure you can do so safely. If you use cannabis, talk to your doctor.

Metronidazole may impair the effectiveness of live bacterial vaccinations, such as the typhoid vaccine. Unless your doctor advises otherwise, do not get any vaccines or vaccinations while taking this drug.

  • thrombocytopenia. Hypersensitivity reactions. Urethral discomfort
  • darkening of urine. Raised liver enzyme values
  • cholestatic hepatitis
  • jaundice. Thrombophlebitis (IV). Potentially Fatal: Anaphylaxis.

Drug Interactions

Alcohol-containing goods, such as cough and cold syrups, aftershave, propylene glycol-containing products, lopinavir solution, and lithium, may interact with this medication.

If you're taking disulfiram or have taken disulfiram in the last three weeks, don't take metronidazole.

Contraindications

Hypersensitivity to metronidazole and other nitroimidazole derivatives in the past. Pregnancy (first trimester) and a mother who breastfeeds her child.

Mode of actions

In vulnerable organisms, metronidazole is reduced to reduction products that interact with DNA to destroy the helical DNA helix and strands, resulting in protein synthesis inhibition and cell death. Most anaerobic protozoa, as well as some gm+ve, gm-ve, and facultative anaerobes, are susceptible to it.

Dosage & Administration

Adults & Children (over 10 years): PO Amoebiasis; Balantidiasis; Blastocystis hominis infection 400-800 mg 3 times/day for 5-10 days. Trichomoniasis 2 g as a single dose for 2 days or 0.6-1 g/day in 2-3 divided doses for 7 days.

Repeat 4-6 wk later if needed. Giardiasis 2 g once daily for 3 consecutive days. Acute dental infections; Acute necrotising ulcerative gingivitis 200 mg 3 times/day. Anaerobic bacterial infections Initial: 800 mg, then 400 mg 8 hrly for about 7 days.

Surgical prophylaxis 400 mg 8 hrly in the 24 hr before surgery followed post-op by IV or rectal route until PO is possible.

Eradication of H. pylori associated w/ peptic ulcer disease W/ omeprazole and amoxicillin: 400 mg 3 times/day for 1 wk. Leg ulcers and pressure sores 400 mg 3 times/day for 7 days. Antibiotic-associated colitis 250-500 mg 2-3 times/day for 10-14 days. Change to oral vancomycin if no clear response after 2 days.

IV: Anaerobic bacterial infections 500 mg 8 hrly. Surgical prophylaxis 500 mg shortly pre-op and repeat 8 hrly. Substitute w/ oral doses at 200 or 400 mg 8 hrly as soon as possible.

Children: IV: 7.5 mg/kg (1.5 ml/kg) 8 hourly.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect even sometimes unable to demonstrate the risk on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required.
