Precautions and warnings

People who are elderly, have an underactive thyroid, kidney or liver problems, head injuries, brain conditions, low blood pressure, shock, adrenal gland problems, asthma, enlarged prostate, bowel diseases (like inflammation or blockage), muscle weakness disorders (like myasthenia gravis), or are infants should use caution.

Side-effects

Drug Interactions

Taking this medication with other drugs that depress the central nervous system (like sedatives, sleeping pills, anesthesia, some antipsychotics, and tranquilizers) can increase their sedative effects. It may also make muscle relaxants stronger. If taken with certain opioid painkillers that act as both agonists and antagonists (like pentazocine, nalbuphine, or buprenorphine), it could reduce pain relief. Taking it with cimetidine can raise drug levels in the blood. This medication may make diuretics less effective by causing the body to retain water. It can also slow down the absorption of mexiletine. It may reduce the effects of GI drugs like cisapride, domperidone, and metoclopramide. When used with drugs that increase dopamine, it could cause very high fever and damage to the nervous system.

**Serious Warning:** Taking this medication with MAO inhibitors can greatly increase its effects, leading to dangerous or even life-threatening problems such as anxiety, confusion, stopped breathing, or coma.

Contraindications

Breathing problems such as slow or shallow breathing, severe asthma, or blocked airways; bowel blockage (paralytic ileus); serious liver disease; patients in a coma; people with high pressure in the skull; or those who are very drunk. Also, lung swelling caused by inhaling a chemical that irritates the lungs.

Mode of actions

Morphine is a type of drug made from phenanthrene that mainly affects the brain, nerves, and smooth muscles. It works by attaching to specific opiate receptors in the brain and changes the way we feel and respond to pain. The feelings of pain relief, happiness, and the risk of addiction are linked to its effects on the mu-1 receptors. Its effects on the mu-2 receptors cause slowed breathing and reduced movement in the intestines. Morphine also produces pain relief in the spine by activating the K (kappa) receptor.

Dosage & Administration

Adult: PO Pain relief 5-20 mg 4 hrly. Intractable cough associated w/ lung cancer As oral soln: Initial: 5 mg 4 hrly.

IV Pain associated w/ MI 10 mg, then a further dose of 5-10 mg if needed.
Acute pulmonary oedema 5-10 mg. Unstable angina unresponsive to anti-ischaemic therapy 2-5 mg repeated every 5-30 mins as needed. Acute pain Initial: 2.5 mg 4 hrly, adjust according to response.

IM/SC Acute pain 10 mg 4 hrly, adjust according to response. Premed in surgical procedures Up to 10 mg 60-90 mins pre-op. Analgesia during labour 10 mg. Intrathecal Moderate to severe pain 0.2-1 mg on a single occasion. Intraspinal Moderate to severe pain Initial: 5 mg epidural inj; after 1 hr, additional doses of 1-2 mg may be given if needed. Total dose: 10 mg/24 hr.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
