Renal or hepatic dysfunction that is mild to moderate (test serum creatinine before treatment, every 3 months for 1st year, every 6 months for the next 4 years, then annually). Patients at risk for pericarditis or myocarditis include the elderly, those with an active peptic ulcer, those who are pregnant or lactating, and those who are predisposed to pericarditis or myocarditis. During treatment, advise patients to report any unexplained bleeding, bruises, purpura, sore throat, fever, or malaise; obtain a blood count and discontinue medication if blood dyscrasias are detected. Advise patients who are taking delayed-release pills to report unbroken or partially broken tablets in their feces on a regular basis. Pyloric stenosis can cause a delay in the passage of food into the colon. Patients with lung disease, especially asthma, should be closely followed throughout their treatment. Sulphasalazine hypersensitivity. Therapy should only be started under medical supervision in patients who have a history of sulphasalazine hypersensitivity. If you experience acute intolerance signs such as abdominal cramps, abrupt abdominal pain, fever, severe headache, or rash, you should stop taking your medication right away. Children's population For a child's effect, there is simply a smattering of evidence (age 6-18 years).
When given lactulose or other medications that lower pH, the release of mesalazine is inhibited. Digoxin absorption may be reduced.
Intolerance to salicylates, aminosalicylates, or any other ingredient in the product. - Severe renal failure (GFR less than 30 mL per minute). - Severe liver disease. - Ulcers of the stomach and duodenum - Infants and toddlers under the age of two.
Sulfasalazine contains the active ingredient mesalamine (5-aminosalicylic acid), although the exact MOA is uncertain; it is thought to suppress prostaglandin and leukotriene synthesis and release in the colon.
For the treatment of mildly to moderately active ulcerative colitis: The usual dosage in adults is two 400 mg tablets to be taken three times a day for a total daily dose of 2.4 grams for a duration of 6 weeks.For the maintenance of remission of ulcerative colitis: The recommended dosage in adults is 1.6 grams daily, in divided doses. Treatment duration in the prospective, well-controlled trial was 6 months.
Child: Dose is dependant on preparation and brand used. Tablets: 5-15 yr: Acute attack: 15-20 mg/kg (max: 1 g) tid; maintenance of remission: 10 mg/kg (max: 500 mg) 2-3 times daily.
Crohn`s disease: 800 mg 3 times daily.
Although animal reproduction studies have shown an adverse effect even sometimes unable to demonstrate the risk on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required.
