Precautions and warnings

Non-infectious pneumonitis is a known side effect of drugs related to rapamycin. In clinical trials, up to 19% of patients who took these drugs developed non-infectious pneumonitis. More severe cases (grade 3) happened in up to 4% of patients, while the most severe cases (grade 4) were seen in up to 0.2% of patients.

Side-effects

Drug Interactions

Drugs that affect CYP3A4 (either inhibitors or inducers), drugs that are CYP2D6 substrates with a narrow therapeutic range, PgP inhibitors, rifampicin, ACE inhibitors, grapefruit juice, and live vaccines.

Contraindications

People who are allergic to this medicine, other similar drugs (rapamycin derivatives), or any of its ingredients should not take it. Allergic reactions can include symptoms such as severe allergic reactions (anaphylaxis), trouble breathing, flushing, chest pain, or swelling of the airways or tongue (angioedema), which may or may not make it hard to breathe. These kinds of reactions have been seen in patients taking everolimus and similar medicines.

Mode of actions

Everolimus is a drug that helps prevent organ rejection after a transplant by stopping the immune system from attacking the new organ. It works in animals like rodents and primates, where it keeps T cells (a type of immune cell) from multiplying. Usually, these T cells start to multiply when they respond to signals from certain molecules called interleukins (like interleukin-2 and interleukin-15). Everolimus blocks the signals inside the T cells that tell them to grow after these interleukins attach to them. As a result, T cells get stuck in an early phase of their growth cycle and cannot multiply.

On a deeper level, everolimus joins with a protein inside cells called FKBP-12. Together, they block another protein called p70 S6 kinase from being activated, which normally happens after a signal from cell growth factors. This blocking happens because everolimus affects the activity of a key regulator protein called FRAP (also known as m-TOR). Since FRAP controls how cells grow and divide, everolimus’ interference with FRAP is what stops the T cells from multiplying.

Dosage & Administration

PO: Advanced HR+ BC, advanced PNET, advanced RCC, or renal angiomyolipoma with TSC:
10 mg once daily with or without food

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
