Precautions and warnings

History of GI disease (such as bleeding or ulcers), high blood pressure, kidney or liver problems, pregnancy, breastfeeding, and old age all increase the risk with this medication.

Monitoring: Blood pressure should be checked closely at the start and during treatment. For patients using ketoprofen for a long time, regular blood tests—including complete blood count, kidney, and liver function tests—should be done.

Side-effects

  • tissue damage at inj site (IM). Cardiovascular toxicity (chest pain

  • weakness
  • slurred speech). May mask the usual signs
  • symptoms of infection. NSAID-induced liver dysfunction
  • anaphylactoid reaction (e.g. difficulty breathing
  • swelling of the face or throat); photosensitivity reactions (topical). Potentially Fatal: Anaphylaxis
  • exfoliative dermatitis
  • Stevens-Johnson syndrome
  • toxic epidermal necrolysis.

Drug Interactions

- It can raise the levels of lithium and methotrexate in your blood.
- It can make blood pressure medicines like ACE inhibitors and angiotensin II blockers less effective.
- Taking it with warfarin increases the risk of stomach bleeding.
- It binds less to proteins when taken with ketoprofen and raises the risk of serious stomach problems when used with aspirin or other NSAIDs.
- Using it with diuretics increases the risk of kidney failure.
- Taking it with corticosteroids makes bleeding and ulcers in the stomach more likely.
- Probenecid can increase its level in your blood.
- Salicylate makes it harder for your body to remove ketoprofen.

Contraindications

Do not use for any route if the person is allergic to aspirin or other NSAIDs, or if they have asthma, swelling (angioedema), hives (urticaria), or nasal allergies (rhinitis). It should also not be used if the person has active stomach or bowel disease (like bleeding or ulcers), severe heart failure, or serious kidney problems. Do not use to treat pain after heart bypass (CABG) surgery.

For rectal use: Do not give to patients with a history of inflammation of the rectum (proctitis) or hemorrhoids.

Mode of actions

Ketoprofen has anti-inflammatory, pain-relieving, and fever-reducing effects. It works mainly by blocking the enzyme cyclooxygenase, which leads to a decrease in the production of prostaglandins.

Dosage & Administration

Adult: PO Rheumatic disorders 100-200 mg/day in 2-4 divided doses. Max: 300 mg/day in divided doses. Pain and inflammation 25-50 mg 6-8 hrly. Max: 300 mg/day.

IM Pain and inflammation associated w/ musculoskeletal and joint disorders; Pain following orthopaedic surgery 50-100 mg by deep inj into the gluteal muscle 4 hrly. Max: 200 mg/24 hr for up to 3 days.

Rectal Rheumatic disorders 100 mg at night. Recommended total dose (combined oral and rectal): Not to exceed 200 mg/day.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
