Precautions and warnings

Treatment with thiazide diuretics at the same time raises the risk of hypercalcemia. In patients receiving digitalis medication, the dosage of calcitriol must be determined with caution, as hypercalcemia might cause cardiac arrhythmias. Hypermagnesemia can be caused by magnesium-containing medications (such as antacids).

Side-effects

  • Weakness; headache; somnolence; nausea; vomiting; dry mouth; constipation; muscle pain; bone pain; metallic taste; polyuria; polydipsia; anorexia; irritability; weight loss; nocturia; mild acidosis; reversible azotemia; generalized vascular calcification; nephrocalcinosis; conjunctivitis (calcific); pancreatitis; photophobia; rhinorrhoea; pruritus; hyperthermia; decreased libido; elevated BUN; albuminuria; hypercholesterolaemia; elevated AST
  • ALT; ectopic calcification; hypertension; cardiac arrhythmias.

Drug Interactions

Calcium: When thiazide diuretics or vitamin D are taken together, milk-alkali syndrome and hypercalcemia might occur. Corticosteroids cause a decrease in absorption. Decreases absorption of tetracyclines, atenolol, iron, quinolones, alendronate, Na fluoride, Zn and calcium-channel blockers. Enhances the effects of digitalis glycosides on the heart and may lead to digitalis intoxication.


Calcitriol: Patients on chronic renal dialysis may develop hypermagnesaemia. Patients on digitalis may develop hypercalcemia, which can lead to cardiac arrhythmias. Cholestyramine and colestipol may inhibit calcitriol absorption in the intestine. Phenytoin, barbiturates may decrease the T1/2 of calcitriol. May develop hypercalcaemia with thiazide diuretics.

Contraindications

Hypercalcemia, evidence of vitamin D toxicity. Pregnancy (dose exceeding RDA). A mother who feeds breast milk to her baby.

Mode of actions

Calcium salt is useful for preventing and treating calcium shortage and negative calcium balance. It's also used as a supplement to help prevent and treat osteoporosis.

Calcitriol increases serum calcium levels by promoting calcium absorption in the intestines and retention in the kidneys. It also stimulates phosphate resorption in the renal tubules, lowering serum phosphatase, PTH, and bone resorption.

Dosage & Administration

Adult: PO Hyperparathyroidism in renal failure 0.25 mcg/day or alternate day. May increase slowly. Hypoparathyroidism/pseudohypoparathyroidism 0.5-2 mcg once daily. Vit D-dependent rickets 0.015-0.02 mcg/kg/day. Maintenance: 0.03-0.06 mcg/kg/day. Max: 2 mcg/day.

IV Hyperparathyroidism in dialysis patients 0.5-4 mcg 3 times/wk. Max: 8 mcg 3 times/wk. Hyperparathyroidism in renal failure 0.5 mcg 3 times/wk, may increase by 0.25-0.5 mcg at 2-4 wk intervals. Maintenance: 0.5-3 mcg 3 times/wk.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
