Precautions and warnings

Be careful when giving this medicine to patients who are allergic to other beta-lactam antibiotics, as they might also react to this drug. Use with caution in people with conditions like epilepsy or problems with their kidneys or liver. Take extra care if the patient is pregnant or breastfeeding.

Drug Interactions

Taking probenecid at the same time as cilastatin can make cilastatin stay in the body longer. Using cilastatin together with ganciclovir can raise the risk of having seizures.

Mode of actions

Imipenem is a strong antibiotic that kills many types of bacteria by stopping them from building their cell walls. It works well because it isn’t broken down by certain enzymes (called ?-lactamases) that many bacteria use to protect themselves. Cilastatin is a medicine that blocks an enzyme in the kidneys (called dehydropeptidase I). When imipenem is given together with cilastatin, it helps prevent imipenem from being broken down in the kidneys and protects against kidney damage. Both drugs are given as sodium salts.

Dosage & Administration

Adult: IV Susceptible infections As imipenem: 1-2 g/day in divided doses 6-8 hrly. Surgical prophylaxis As imipenem: 1 g on induction of anesth, followed by 1 g 3 hr later. Additional doses of 500 mg may be given at 8 and 16 hr after induction if needed.

Child: >40 kg: same as adult dose. >3 mth and

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
