Precautions and warnings

Hypercalcemia caused by an unknown reason. Pediatric doses must be tailored to the particular child and closely monitored by a physician. Coronary artery disease, renal failure, and arteriosclerosis are all common in the elderly. Hypoparathyroidism.

Because an excessive dose of Calcitriol causes hypercalcemia and, in rare cases, hypercalciuria, serum calcium should be measured twice weekly early in treatment while dosage is being adjusted.
Calcitriol users with normal renal function should avoid becoming dehydrated. It's important to drink enough of water.

Side-effects

  • Adverse events associated with Calcitriol therapy are similar to those encountered with excess vitamin D intake

Drug Interactions

Treatment with thiazide diuretics at the same time raises the risk of hypercalcemia. In patients receiving digitalis medication, the dosage of calcitriol must be determined with caution, as hypercalcemia might cause cardiac arrhythmias. Hypermagnesemia can be caused by magnesium-containing medications (such as antacids).

Contraindications

Hypercalcaemia, evidence of vitamin D toxicity. Pregnancy (dose exceeding RDA). A mother who feeds breast milk to her baby.

Mode of actions

Calcitriol increases serum calcium levels by promoting calcium absorption in the intestines and retention in the kidneys. It also stimulates phosphate resorption in the renal tubules, lowering serum phosphatase, PTH, and bone resorption.

Dosage & Administration

Adult: Capsule: The optimal daily dose of Calcitriol must be carefully determined for each patient on the basis of the serum calcium level. Calcitriol can be administered orally either 0.25 mcg or 0.50 mcg.

Postmenopausal osteoporosis
The recommended dosage is 0.25 mcg twice daily or 0.50 mcg once daily. Serum creatinine levels should be determined at 4 weeks, 3 and 6 months and 6 monthly intervals thereafter.

Renal osteodystrophy (Dialysis patients)
The recommended initial dose of Calcitriol is 0.25 mcg/day. Patients with normal or only slightly reduced serum calcium levels may respond to Calcitriol doses of 0.25 mcg every other day. Most patients undergoing hemodialysis respond to doses between 0.5 and 1 mcg/day.

Hypoparathyroidism & Rickets
The recommended initial dosage of Calcitriol is 0.25 mcg/day given in the morning. If a satisfactory response is not observed, the dose may be increased at 2 to 4 week intervals.

Predialysis patients:
The recommended initial dosage of Calcitriol is 0.25 mcg/day in adults and pediatric patients 3 years of age and older. This dosage may be increased if necessary to 0.5 mcg/day.
Child dose: 1 month - 3 years of age, the recommended initial dosage of Calcitriol is 10 to 15 ng/kg/day.

Injection
The recommended intravenous initial dose of Calcitriol injection, depending on the severity of the hypocalcemia and/or secondary hyperparathyroidism, is 1 mcg (0.02 mcg/kg) to 2 mcg administered three times weekly, approximately every other day. Doses as small as 0.5 mcg and as large as 4 mcg three times weekly have been used as an initial dose. If a satisfactory response is not observed, the dose may be increased by 0.5 to 1 mcg at two to four week intervals.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
