Patients with mild to severe renal failure or mild hypercalciuria need to be closely monitored. Patients with mild to moderate renal failure or mild hypercalciuria should have their plasma calcium levels and urinary calcium excretion checked on a regular basis. Urinary Calcium excretion should be assessed in patients with a history of renal stones to rule out hypercalciuria. Long-term treatment should be monitored for serum and urine calcium levels, as well as kidney function, and medication should be reduced or stopped if urinary calcium reaches 7.5 mmol/24 hours. Calcium carbonate: When used with thiazide diuretics or Vitamin D, milk-alkali syndrome and hypercalcemia might occur. Corticosteroids cause a decrease in absorption. Tetracyclines, atenolol, iron, quinolones, alendronate, Na fluoride, Zn, and calcium-channel blockers are all reduced in absorption. Calcium Lactate: Calcium-channel blockers' efficacy may be reduced by this substance. Concurrent administration of IV calcium salt and cardiac glycosides can result in significant side effects. Calcium gluconate: When high calcium doses are combined with thiazide diuretics, milk-alkali syndrome and hypercalcemia can occur. Digoxin toxicity may be exacerbated. Calcium-channel blockers' effects are reduced. Calcitriol improves absorption (a Vitamin-D metabolite).
When used with thiazide diuretics, calcium, or phosphate, vitamin D3 increases the risk of hypercalcemia. Antiepileptic medications (such as carbamazepine, phenobarbitone, phenytoin, and primidone) can raise Vitamin D requirements. Vitamin-D efficacy may be reduced by antibiotics such as rifampicin and isoniazid. Vitamin D may be counteracted by corticosteroids. Any cardiac glycoside, such as digoxin.
