Precautions and warnings

Problems with kidney function can occur. There is a risk of high potassium levels, low blood volume, collagen vascular disease, and valve narrowing. Issues may arise during, right before, or right after anesthesia, or if there is narrowing in one of the kidney arteries. Low blood pressure may happen if you are dehydrated or taking diuretics. White blood cell count should be monitored. Use with caution during breastfeeding and in children.

Drug Interactions

This medication may increase the blood pressure-lowering effects of diuretics, other hypertension medicines, certain antidepressants (TCAs), nitrates, or anesthesia. Taking it together with lithium can raise the risk of lithium toxicity. Using it with NSAIDs may increase the risk of kidney problems. Taking it with potassium-sparing diuretics or potassium supplements can also raise the risk of high potassium levels in the blood.

Contraindications

Do not use if you are allergic or have had swelling (angioedema) from ACE inhibitors before. This medication should also not be used if you have narrowing of both kidney arteries (bilateral renal artery stenosis) or if you are pregnant.

Mode of actions

Enalapril is a medication that starts off inactive and is then changed in the body to its active form, enalaprilat. It works by blocking the ACE enzyme, which normally turns angiotensin I into angiotensin II—a chemical that narrows blood vessels. By blocking this process, enalapril lowers the amount of angiotensin II, leading to more relaxed blood vessels and lower blood pressure. It also reduces the hormone aldosterone, which usually makes the body hold on to salt and water. As a result, enalapril helps widen blood vessels and lower blood pressure.

Dosage & Administration

Adult: PO HTN Initial: 5 mg/day at bedtime. Maintenance: 10-20 mg once daily. Severe: Up to 40 mg/day in divided doses. Max: 40 mg/day.

Heart failure Initial: 2.5 mg/day. Maintenance: 20 mg once daily, up to 40 mg/day in 2 divided doses.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
