Precautions and warnings

Enlarged prostate, liver or kidney problems, gout, diabetes, or difficulty urinating should be taken into consideration. The infusion rate should not go over 4 mg per minute to avoid the risk of ear toxicity. Monitor the patient's fluid and electrolyte levels as well as kidney function. This medication can lower the amounts of calcium and magnesium in the blood, so these should be checked regularly. Use with caution during pregnancy and breastfeeding.

Drug Interactions

Analgesic medicines can reduce the sodium-removing effect of furosemide. They can also interfere with medications for lowering blood sugar and for treating gout. When taken with the blood pressure medicine diazoxide, furosemide may raise blood sugar levels. It can make muscle relaxants work less well. Using furosemide with aminoglycoside antibiotics, especially if you have kidney problems, increases the risk of hearing loss. It may also worsen kidney damage caused by cephalosporin antibiotics. Furosemide can make blood pressure medicines stronger, but its effect is reduced by corticosteroids. Some drugs like phenytoin and indomethacin can lower how well furosemide works.

**Warning:**
Using furosemide with ACE inhibitors can cause dangerously low blood pressure. NSAIDs (like ibuprofen) can stop furosemide from lowering blood pressure or removing excess water. The risk of abnormal heartbeats increases when furosemide is combined with cardiac glycosides.

Contraindications

Do not use this medicine if you have any of the following conditions: severe loss of sodium and water from your body, allergy to sulphonamides or furosemide, low potassium (hypokalaemia), low sodium (hyponatraemia), are in a pre-coma state due to liver cirrhosis, are unable to produce urine (anuria), have kidney failure, or have Addison’s disease.

Mode of actions

Furosemide works by blocking the reabsorption of sodium and chloride, mainly in the ascending part of the Loop of Henle in the kidney. This causes the body to lose more potassium and ammonia in the urine, but it decreases the excretion of uric acid. Furosemide also increases the level of plasma renin, which can sometimes lead to a condition called secondary hyperaldosteronism. It helps lower blood pressure in both people with high blood pressure and those with normal blood pressure. Additionally, furosemide can help reduce fluid build-up in the lungs (pulmonary edema) even before it starts increasing urine output.

Dosage & Administration

Adult: PO HTN 40-80 mg/day. Oedema Initial: 20 mg/day. Max: 600 mg/day in severe cases.

IV Pulmonary oedema 40 mg, if response is inadequate w/in 1 hr, a further dose of 80 mg may be given.

Oliguria in acute or chronic renal failure For GFR: 5-20 mL/min: Infuse 250 mg over 1 hr. If urine output is insufficient w/in the next hr, may follow by 500 mg infused over 2 hr. If urine output is still unsatisfactory w/in 1 hr after the 2nd infusion, a 3rd dose of 1 g may be infused over 4 hr. Rate of infusion: 50 mg must be given as IV infusion.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
