Precautions and warnings

Be careful when using neuromuscular blocking agents at the same time, or in people with myasthenia gravis, Parkinson’s disease, or conditions that increase the risk of ear or kidney damage. Use caution during breastfeeding. Check gentamicin blood levels in patients who get high doses or long-term treatment, as well as in infants, elderly patients, those with kidney problems, cystic fibrosis, or severe obesity. Regularly monitor hearing and kidney function.

Side-effects

  • Dizziness or vertigo; acute renal failure
  • interstitial nephritis
  • acute tubular necrosis; electrolyte imbalances; transient elevation of serum bilirubin
  • aminotransferases; purpura; nausea
  • vomiting; convulsions
  • mental depression
  • hallucinations. Atrophy or rat necrosis at inj sites. Potentially Fatal: Nephrotoxicity
  • ototoxicity
  • neuromuscular blockade (may unmask or aggravate myasthaenia gravis).

Drug Interactions

This medicine works well with ampicillin, benzylpenicillin, and other beta-lactam antibiotics. There is a higher risk of serious breathing problems if used with anesthetics or opioid painkillers. It can also reduce the removal of zalcitabine by the kidneys and may cause low calcium levels if taken with bisphosphonates. Do not use it with agalsidase alfa or beta, as it can block the action of ?-galactosidase.

**Potentially Fatal Effects:**
- The risk of ear damage (ototoxicity) increases if used with ethacrynic acid or furosemide.
- The risk of kidney damage (nephrotoxicity) increases when combined with cephalosporins, ciclosporin, cisplatin, vancomycin, hydrocortisone, or indometacin.
- It can strengthen the effects of drugs that block nerve-muscle signals (neuromuscular blocking agents).

Contraindications

Do not use if you have ever had an allergic reaction to aminoglycosides, are pregnant, have liver problems, or have a perforated eardrum.

Mode of actions

Gentamicin is a type of antibiotic called an aminoglycoside. It works by attaching to the 30s and 50s parts of the bacteria’s ribosomes, which are responsible for making proteins. This stops the bacteria from making the proteins they need and damages their cell membrane, making it easier to kill them.

Dosage & Administration

Adult: As a 0.1% cream; Apply to the affected area 3-4 times daily.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
