Precautions and warnings

Phaeochromocytoma, stable heart failure, and nonallergic bronchospasm: use with caution. Do not stop taking this medication suddenly. Use carefully in people with diabetes or liver problems—liver function tests may be needed. Be cautious in elderly patients, and during pregnancy or breastfeeding.

Drug Interactions

Halothane can strengthen the blood pressure-lowering effect. Cimetidine increases how much of the drug is absorbed by the body, while glutethimide decreases it. Nitroglycerin can add to the blood pressure-lowering effect. Using tricyclic antidepressants (TCAs) may cause more tremors. Taking calcium channel blockers (such as verapamil or diltiazem) together increases the risk of slow heart rate and heart block.

Contraindications

Do not use in people who have:
- 2nd or 3rd degree heart block
- Cardiogenic shock
- Obstructive airway diseases, such as asthma
- Uncontrolled heart failure
- Very slow heart rate (severe bradycardia)
- Sick sinus syndrome
- Prinzmetal's angina
- Severe problems with blood flow to the arms or legs (peripheral arterial disease)

Mode of actions

Labetalol works by blocking adrenergic stimulation of beta-receptors in the heart, airways, and blood vessels, as well as alpha1-receptors in blood vessels. It also has some mild beta2-agonist effects and can help stabilize cell membranes.

Dosage & Administration

Adult: PO HTN Initial: 100 mg twice daily, up to 200-400 mg twice daily. Max: 2.4 g/day in 2-4 divided doses.

IV Emergency treatment of HTN 50 mg, repeat at 5-min intervals if needed, up to a total dose of 200 mg. HTN in pregnancy Initial: 20 mg/hr, double dose every 30 mins until a favourable response or a dose of 160 mg/hr is reached.

HTN after MI Initial: 15 mg/hr, may increase slowly until a favourable response or a dose of 120 mg/hr is achieved.

Hypotensive anesth Initial: 10-20 mg, may increase slowly if satisfactory hypotension is not reached after 5 mins.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
