**Some drugs and medicines can affect how well this medication is absorbed or how much of it is in your body.**
- Medicines like antimuscarinics, antacids, H2-blockers, PPIs, and sucralfate can lower how much of this drug is absorbed.
- Other drugs such as rifampicin, isoniazid, efavirenz, nevirapine, and phenytoin can reduce the amount of this medication in your bloodstream.
- This medication can also lower the levels of isoniazid and rifampicin.
- It may make oral contraceptives (birth control pills) less effective.
- It can raise the blood levels of certain drugs that are broken down by the CYP3A4 enzyme, including digoxin, some blood thinners, sildenafil, and tacrolimus.
**Serious and potentially fatal interactions:**
- This medication can increase and prolong the sedative (sleepy) effects of midazolam and triazolam.
- It can increase the amount and effects of certain drugs (like astemizole, cisapride, dofetilide, pimozide, quinidine, and terfenadine), raising the risk of dangerous heart rhythm problems (torsade de pointes).
- There is a higher risk of muscle problems (myopathy) if taken with certain cholesterol-lowering drugs (like lovastatin and simvastatin).
- It can greatly increase blood levels of nisoldipine.
- There is a higher risk of high potassium levels and low blood pressure if used with eplerenone.
- It increases the risk of blood vessel spasms that could cause a stroke when used with certain migraine drugs (ergot alkaloids such as ergotamine and dihydroergotamine).
Do not use this medication if you are allergic to it, have existing liver problems, or have porphyria. Also, do not take it at the same time as cisapride, terfenadine, or astemizole.
Ketoconazole works by blocking a fungal enzyme called CYP450, which disrupts the production of important fats (triglycerides and phospholipids) in the fungus. This changes the permeability of the fungal cell membrane, making it easier for substances to leak in and out. Ketoconazole also blocks other fungal enzymes, causing harmful levels of hydrogen peroxide to build up inside the fungus.
Adult: PO Fungal infections 200 mg once daily, up to 400 mg once daily if needed. Usual duration: 14 days and for at least 1 wk after symptoms have cleared and cultures have become negative.
Chronic vag candidiasis 400 mg once daily for 5 days.
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
