Precautions and warnings

People with severe kidney or liver disease, allergies or asthma, lupus, acute gout, diabetes, advanced age, or collagen vascular diseases should stop taking this medicine before any tests for parathyroid function. This medicine should also not be used during pregnancy or breastfeeding.

Drug Interactions

Hydrochlorothiazide can make low blood pressure from standing up (orthostatic hypotension) worse when taken with alcohol, sleeping pills (barbiturates), or narcotics. When used with other blood pressure medicines, their effects add up. Medicines like cholestyramine and colestipol can lower how much hydrochlorothiazide your body absorbs. Using it with steroids can cause low potassium levels. It can also make muscle relaxants like tubocurarine stronger. Hydrochlorothiazide lowers how quickly the body clears lithium, which can raise the risk of lithium poisoning. Pain relievers called NSAIDs can make hydrochlorothiazide less effective.

Taking ramipril with certain water pills called potassium-sparing diuretics can cause dangerously high potassium levels. Ramipril can also raise lithium levels in the blood. NSAIDs (painkillers) can make ramipril less effective and can also harm kidney function.

Contraindications

Do not use in people with no urine output (anuria), those who are allergic to the drug (hypersensitivity), those who have had angioedema (swelling under the skin), or in people with narrowing of the arteries to both kidneys (bilateral renal artery stenosis) or to one kidney (unilateral renal artery stenosis).

Mode of actions

Ramipril is a long-acting ACE inhibitor that turns into its active form, ramiprilat, in the body. It lowers blood pressure when you're lying down and standing, without causing your heart to beat faster. Ramipril can also help reverse thickening of the heart’s walls and increase blood flow to the kidneys. In people with heart failure, it reduces pressure in the lungs and heart, helping the heart work better.

Hydrochlorothiazide is a type of diuretic that helps your body get rid of extra sodium and chloride through urine. This also causes you to lose some potassium and bicarbonate.

When used together, these two medicines work better to lower blood pressure than when used alone. They also have the benefit of being taken just once a day.

Dosage & Administration

Adult: PO Per tab contains ramipril 2.5 mg and hydrochlorthiazide 12.5 mg: 1 tab/day. Max: 4 tab/day.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
