Precautions and warnings

People with problems like fluid and electrolyte imbalances, liver cirrhosis, severe heart failure, high uric acid, or mild to moderate kidney problems need special care. This is especially important for older adults. It’s important to regularly check fluid and electrolyte levels. Extra caution is needed if you have kidney or liver disease, diabetes, gout, high cholesterol, or irregular heartbeats.

Drug Interactions

NSAIDs can reduce the effectiveness of blood pressure medicines. They also decrease the action of oral blood thinners by lowering prothrombin levels. Using corticosteroids at the same time increases the risk of low potassium (hypokalaemia).

Serious Effects:
- Diuretics can make bone marrow suppression from cancer medicines worse.
- If diuretics cause too much fluid loss, this can increase the risk of kidney damage from aminoglycoside antibiotics.
- Diuretics can make diabetes medicines less effective.
- They can increase the toxicity of digitalis (heart medicine) due to low potassium levels.
- Losing too much fluid can also make lithium (used in mental health treatment) more toxic, but suddenly stopping diuretics can lower lithium to ineffective levels.
- Low potassium from diuretics can cause long-lasting paralysis when used with tubocurarine (a muscle relaxant).

Contraindications

Do not use in people with serious kidney problems or who cannot produce urine, severe liver problems, Addison’s disease, high calcium levels in the blood, or asthma. Also avoid if you are allergic or have had a severe allergic reaction before. This medicine should not be used during pregnancy or breastfeeding.

Mode of actions

Chlortalidone is a long-lasting medication taken by mouth to lower blood pressure and help the body get rid of extra fluid. It belongs to a group of medicines called diuretics, which work by helping the kidneys remove more salt (sodium), chloride, and water from the body. Chlortalidone mainly acts on a part of the kidneys called the cortical diluting segment in the loop of Henle, where it blocks sodium from being reabsorbed.

Dosage & Administration

Adult: PO HTN 25 mg/day, up to 50 mg/day.
Oedema Initial: 25-50 mg/day up to 100-200 mg/day in severe cases. Maintenance: 25-50 mg/day or on alternate days.
Diabetes insipidus Initial: 100 mg twice daily. Maintenance: 50 mg/day.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
