Precautions and warnings
**重症肌無力、頭部受傷、顱骨內壓力升高、腦損傷、腎臟或肝臟問題、新生兒和不習慣阿片類藥物的患者應謹慎使用這種藥物。 **
老年人、健康狀況不佳的人或氧氣含量低或二氧化碳含量高的人出現呼吸問題的風險更大。 如果患者患有甲狀腺問題、前列腺肥大、腸道炎症、心率緩慢或心律異常，請格外小心。
通過靜脈注射太快可能會使胸部和肌肉僵硬，導致呼吸困難甚至停止呼吸。 長期使用可能會讓身體習慣，產生身體和精神上的依賴性。 長期使用後突然停止可能會導致戒斷症狀。
如果在母乳喂養期間使用，需要小心。 懷孕期間避免大劑量和長期使用。
Side-effects
- 噁心、嘔吐；心動過緩、水腫、中樞神經系統抑制、精神錯亂、頭暈、嗜睡、頭痛、鎮靜、短暫性低血壓、外周血管舒張；顱內壓增高。 高靜脈注射劑量可能會導致胸壁僵硬。 經皮：皮疹、紅斑和瘙癢。 潛在致命：呼吸抑制、軀幹強直、喉痙攣、支氣管收縮。
Drug Interactions
芬太尼與 CYP3A4 抑製劑（如紅黴素、克拉黴素、某些抗真菌藥、利托那韋、胺碘酮、奈法唑酮、阿瑞吡坦、地爾硫卓和維拉帕米）一起服用會升高血液中芬太尼的濃度，從而可能導致危險或致命的呼吸問題。 如果芬太尼與 SSRIs、SNRIs 或 MAOIs 一起使用，發生稱為血清素綜合徵的嚴重疾病的風險也會更高。 利福黴素等藥物可能會降低芬太尼的水平。 將芬太尼與全身麻醉劑、鎮靜劑、巴比妥類藥物或其他麻醉劑一起使用可以增強其鎮靜作用。 氯化銨可能會使芬太尼更快地離開體內，而吩噻嗪類藥物則可以使芬太尼的降血壓作用更強。 芬太尼也可能降低培維索孟的有效性。
Mode of actions

Dosage & Administration
成人：PO 突發性癌症疼痛 作為 loz：最初，在 15 分鐘內服用 200 微克，以緩解突發性疼痛；如果需要，可以在 15 分鐘後重複一次。 每天不超過 4 單位劑量。
IV 輔助全身麻醉 患者進行自發呼吸：初始劑量：50-200 mcg，補充 50 mcg。 使用輔助通氣的患者：初始：300-3,500 mcg（最高 50 mcg/kg），根據反應補充 100-200 mcg。
麻醉前肌注 Premed 50-100 mcg，麻醉誘導前 30-60 分鐘。
經皮治療 頑固性癌症疼痛 初始：在未使用阿片類藥物的患者中使用每小時 25 微克芬太尼的貼片。 根據反應調整劑量。 對於已接受強效阿片類藥物治療的患者，初始劑量應基於之前 24 小時阿片類藥物的需求量。 每隔 72 小時更換補丁並將新補丁應用到不同的站點。
Pregnancy & Lactation
儘管動物生殖研究顯示對胎兒有不良影響，並且在孕婦中沒有充分且良好對照的研究，但在懷孕期間，儘管存在潛在風險，但在確實需要時可以使用該藥物。
在懷孕最後 3 個月內使用這種藥物的新生兒母親可能很少會出現困倦、肌肉僵硬或顫抖、餵養或呼吸困難、持續哭泣等症狀。 如果您發現寶寶有任何這些症狀，尤其是在第一個月內，請立即告訴醫生。
