Precautions and warnings

**Myasthenia gravis, head injury, increased pressure inside the skull, brain lesions, kidney or liver problems, newborns, and patients not used to opioids should use caution with this medication.**

Older adults, people in poor health, or those with low oxygen or high carbon dioxide levels are at greater risk for breathing problems. Use extra care if the patient has thyroid problems, an enlarged prostate, bowel inflammation, a slow heart rate, or abnormal heart rhythms.

Giving this medicine too quickly by IV can make the chest and muscles stiff, leading to trouble breathing or even stopping breathing. Using it for a long time may cause your body to get used to it, and both physical and mental dependence can develop. Stopping suddenly after long-term use may lead to withdrawal symptoms.

Caution is needed if using during breastfeeding. During pregnancy, avoid high doses and long-term use.

Drug Interactions

Taking fentanyl with CYP3A4 inhibitors (such as erythromycin, clarithromycin, certain antifungals, ritonavir, amiodarone, nefazodone, aprepitant, diltiazem, and verapamil) can raise fentanyl levels in the blood, which may lead to dangerous or fatal breathing problems. There is also a higher risk of a serious condition called serotonin syndrome if fentanyl is used with SSRIs, SNRIs, or MAOIs. Drugs like rifamycins may lower fentanyl levels. Using fentanyl with general anesthetics, tranquilizers, barbiturates, or other narcotics can increase its sedative effects. Ammonium chloride may make fentanyl leave the body faster, while phenothiazines can make fentanyl’s blood pressure-lowering effect stronger. Fentanyl may also reduce the effectiveness of pegvisomant.

Mode of actions

Fentanyl is a strong painkiller that works by attaching to specific receptors in the brain and nervous system. This changes how your body senses pain, raises your ability to tolerate pain, and blocks pain signals from traveling up the nerves to the brain.

Dosage & Administration

Adult: PO Breakthrough cancer pain As a loz: Initially, 200 mcg over 15 minutes for an episode of breakthrough pain; may repeat once after 15 minutes if needed. Not more than 4 unit doses/day.

IV Adjunct to general anesth Patients w/ spontaneous resp: Initial: 50-200 mcg, w/ supplements of 50 mcg. Patients w/ assisted ventilation: Initial: 300-3,500 mcg (up to 50 mcg/kg), w/ supplements of 100-200 mcg depending on response.

IM Premed before anesth 50-100 mcg 30-60 mins before induction of anesth.

Transdermal Intractable cancer pain Initial: Apply patch that delivers ?25 mcg/hr of fentanyl in opioid-naive patients. Adjust dose according to response. For patients who have been receiving a strong opioid, initial dose should be based on the previous 24-hr opioid requirement. Replace patch 72 hrly and apply new patch to a different site.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
