Precautions and warnings

Liver problems, blocked bile ducts, kidney issues, and narrowed kidney arteries can affect treatment. Make sure the body isn’t dehydrated before starting therapy. Check blood potassium levels often, especially in older adults and people with kidney problems.

Drug Interactions

Some medications and substances can affect potassium levels or interact with other drugs. These include lithium, diuretics that either increase (kaliuretic) or decrease (potassium-sparing) potassium, laxatives, corticosteroids, ACTH, amphotericin, carbenoxolone, penicillin G, salicylates, potassium supplements, and potassium salt substitutes. Other drugs that can raise potassium (cause hyperkalemia) include digitalis, antiarrhythmics, and those known to cause torsades de pointes. Additional substances that may interact include alcohol, barbiturates, narcotics, diabetes medications (oral antidiabetics and insulin), anion-exchange resins, NSAIDs, certain muscle relaxants, pressor amines, allopurinol, calcium salts, beta-blockers, diazoxide, anticholinergics, amantadine, and chemotherapy drugs (cytotoxics).

Contraindications

Severe liver problems, blocked bile ducts, pregnancy, and breastfeeding.

Mode of actions

Hydrochlorothiazide works by stopping the reabsorption of sodium and chloride in the kidney’s distal tubules. This makes the body get rid of more sodium, water, potassium, and hydrogen ions in the urine.

Telmisartan is a type of drug called an angiotensin II receptor blocker (ARB). It lowers blood pressure by blocking angiotensin II from attaching to its AT1 receptors. This prevents angiotensin II from narrowing blood vessels and causing the body to release aldosterone.

Dosage & Administration

Adult: PO 80-160 mg telmisartan/12.5-25 mg once daily.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
