Precautions and warnings
People with kidney or liver problems, cirrhosis, diabetes, low blood sodium, high uric acid, a history of kidney stones, or low folic acid levels should use caution. Regularly check electrolyte levels, especially potassium. Watch for signs of liver problems, blood disorders, or unusual reactions. Stop taking the medication if a kidney stone develops. Extra care is needed during pregnancy and in elderly patients.
Side-effects
- Photosensitivity reactions; increase in uric acid concentrations; megaloblastic anaemia; thrombocytopenia; hyperkalaemia; nephrolithiasis; renal failure; GI disturbances such as nausea
- vomiting
- diarrhoea; dry mouth; fatigue; dizziness; skin rash
- muscle cramps; headache; orthostatic hypotension; increased BUN concentrations
- LFT abnormalities.
Drug Interactions
It may increase the harmful effects of digitalis glycosides, the muscle-relaxing effects of competitive muscle relaxants, and the impact of medications that lower blood pressure. The risk of low blood pressure when standing up (postural hypotension) is higher if taken with alcohol, barbiturates, or opioids. The potassium-lowering effect is stronger when combined with corticosteroids, lithium, ACTH, or carbenoxolone.
Contraindications
People who have severe kidney or liver problems, who already have high potassium levels (hyperkalemia) or develop it from taking medication, or who are at risk for breathing problems or metabolic acidosis should avoid this. It should also not be used during breastfeeding.
Mode of actions
Hydrochlorothiazide works by blocking the reabsorption of sodium and chloride in the kidney’s distal tubules. This leads to the body removing more sodium, water, potassium, and hydrogen ions through urine.
Triamterene is a potassium-sparing diuretic that mainly acts on the kidney’s distal tubules. It helps the body lose more sodium while keeping more potassium, lessening the potassium loss caused by other diuretics. Triamterene is similar in structure to folate and can interfere with folate activity.
Dosage & Administration
Oedema
Adult: 150-250 mg/day in 2 divided doses, after breakfast and lunch. Maintenance: doses may be given on alternate days. Max: 300 mg/day.
Renal impairment: Use with caution. Severe: avoid.
Hepatic impairment: Severe: avoid.
Hypertension
Adult: In combination with other diuretics e.g. hydrochlorothiazide: Initially, 50 mg daily.
Renal impairment: Use with caution. Severe: avoid.
Hepatic impairment: Severe: avoid.
Pregnancy & Lactation
This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.
There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
