Precautions and warnings

Use NSAIDs at the lowest dose and for the shortest time needed. NSAIDs can increase the risk of serious heart problems like heart attack and stroke, especially if you use them for a long time or already have heart disease or risk factors. They can also raise blood pressure or make existing high blood pressure worse. NSAIDs can cause your body to hold onto fluid, which can be dangerous if you have heart failure.

Serious stomach and intestine problems can happen with NSAIDs, such as inflammation, ulcers, bleeding, and even holes in the stomach or intestines. The risk is higher if you have a history of stomach problems; use aspirin, blood thinners, steroids; smoke; drink alcohol; are elderly; or have poor overall health.

NSAIDs may cause anemia, so patients taking them long-term should have their blood checked regularly. They can also stop blood from clotting normally, increasing the risk of bleeding.

Rare but serious liver damage has happened, including jaundice, severe hepatitis, liver failure, and liver death. NSAIDs can harm the kidneys, especially in people who already have kidney, heart, or liver problems, or who are elderly, dehydrated, or taking certain other drugs. Do not use NSAIDs if you have severe kidney disease.

People with asthma should be careful with NSAIDs, and avoid them if they have ever had asthma triggered by aspirin. NSAIDs can cause serious skin reactions, such as rashes, peeling skin, and rare but life-threatening conditions like Stevens-Johnson syndrome. Rare cases of meningitis (aseptic meningitis) in lupus patients, and pancreatitis, have also been reported.

How NSAIDs are processed in children has not been well studied. Do not use NSAIDs in the third trimester of pregnancy. Be careful if using them while breastfeeding.

Drug Interactions

Using this medication together with dimethyl sulfoxide can cause nerve damage in the arms and legs (peripheral neuropathy). It can also increase the risk of kidney damage when taken with cyclosporine, increase the chance of methotrexate toxicity, and raise the risk of bleeding if used with other NSAIDs, warfarin, or drugs that dissolve blood clots. It may reduce the effectiveness of blood pressure medications and has unpredictable interactions with lithium.

Contraindications

Do not use if you are allergic to this medication. Avoid during the third trimester of pregnancy or around the time of surgery for coronary artery bypass graft (CABG).

Mode of actions

Sulindac is a medication that needs to be changed by the body into its active form, called a sulfide metabolite. It works by blocking an enzyme called cyclooxygenase, which leads to less production of prostaglandins, substances that cause inflammation and pain.

Dosage & Administration

Adult: PO Symptomatic treatment of rheumatoid arthritis, osteoarthritis, or ankylosing spondylitis 150 mg twice daily.
Acute painful shoulder 200mg twice daily; usually for 7-14 days.
Acute gouty arthritis200 mg twice daily; usually for 7 days. Max: 400mg/day.

Hepatic impairment: Acute and chronic hepatic disease: May need dosage reduction.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
