Precautions and warnings

Tolterodine should be used carefully in people who have a risk of urinary retention, slow digestion, kidney or liver problems, or myasthenia gravis.

Side-effects

Drug Interactions

There is a higher chance of overdose if you take this medicine with strong CYP3A4 inhibitors—these include certain antibiotics (like erythromycin and clarithromycin), antifungal drugs (such as ketoconazole and itraconazole), protease inhibitors, ciclosporin, or vinblastine. If you use drugs that can prolong the QT interval, such as some heart rhythm medications (class Ia and class III antiarrhythmics), there is a greater risk of serious heart problems like torsade de pointes. Taking this medicine with centrally acting anticholinesterases (like donepezil or rivastigmine) can increase the risk of ‘cholinergic neurogenic hypersensitivity’. Finally, using this medicine together with other antimuscarinic drugs raises the risk of side effects caused by antimuscarinic action.

Contraindications

Tolterodine should not be used by people who are allergic to tolterodine or any ingredient in the medicine. It is also not safe for people who have trouble emptying their bladder (urinary retention) or have uncontrolled narrow angle glaucoma.

Mode of actions

Tolterodine is a medicine that works by blocking muscarinic receptors, much like atropine does. It is used to treat symptoms such as urge urinary incontinence (sudden, strong need to urinate), frequent urination, and urgent need to urinate.

Dosage & Administration

Oral
Overactive bladder
Adult: >18 yr: As immediate release preparation: 2 mg bid, reduce to 1 mg bid if needed to reduce side effects. Reduce to 1 mg bid in patients concurrently taking CYP3A4 inhibitors. As extended release preparation: 4 mg once daily, may reduce to 2 mg daily if needed to reduce side effects. Reduce to 2 mg daily in patients concurrently taking CYP3A4 inhibitors.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
