Some patients may have signs of systemically active corticosteroid withdrawal during oral corticosteroid withdrawal, such as joint and/or muscular discomfort, lassitude, and depression, despite maintaining or even improving respiratory function. Budesonide inhalers can commonly treat asthma symptoms with less hypothalamic-pituitary-adrenal (HPA) suppression than therapeutically similar oral Prednisone dosages. Because Budesonide is taken into the bloodstream and can be systemically active at larger doses, the full benefit of the Budesonide inhaler in reducing HPA dysfunction can only be expected if suggested dosages are not exceeded and individual patients are titrated to the lowest effective dose. Physicians should take this into account when administering a Budesonide inhaler because individual sensitivity to impacts on cortisol production exists.
Increased systemic exposure combined with CYP3A4 inhibitors of high potency (for example, ketoconazole). CYP3A4 inducers and decreased systemic exposure (for example, carbamazepine).
In the primary treatment of status asthmaticus or other acute episodes of asthma requiring extensive therapy, budesonide inhalation aerosol is contraindicated. The use of a Budesonide inhaler is not recommended if you have a Budesonide hypersensitivity.
Budesonide inhibits protein synthesis, inhibits the migration of polymorphonuclear leukocytes and fibroblasts, reverses capillary permeability, and stabilizes lysosomes at the cellular level, all of which help to avoid or regulate inflammation.
Adult: Inhalation Asthma As metered dose inhaler: 400 mcg/day in 2 divided doses, up to 1.6 mg/day in severe cases. Maintenance: 200-400 mcg/day. As dry powd inhaler: 200-800 mcg/day. As nebulised soln: Inhale. Adults and elderly: 1 - 2 mg twice daily.
Children 3 months to 12 years of age: 0.5 - 1 mg twice daily.
Although animal reproduction studies have shown an adverse effect even sometimes unable to demonstrate the risk on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required.
