People who may get high potassium levels (hyperkalaemia) or acidosis should be monitored closely. Check blood electrolyte levels regularly. Use with caution in patients who have kidney or liver problems, diabetes, or who are elderly. Take extra care during pregnancy.
Aspirin may prevent the body from getting rid of sodium. This medicine might also make carbenoxolone less effective at healing ulcers. Using it along with NSAIDs or ciclosporin can increase the risk of kidney damage. There is a chance of high potassium levels (hyperkalaemia) if taken with potassium supplements, ACE inhibitors, angiotensin II blockers, NSAIDs, ciclosporin, or trilostane. The risk of low blood pressure when standing up (orthostatic hypotension) is higher if used with barbiturates, narcotics, or alcohol. This medicine may also make blood vessels less responsive to drugs that raise blood pressure. It can also increase how long digoxin stays in the body.
Serious warning: Taking this medicine with lithium may increase the risk of dangerous lithium toxicity.
No urine production (anuria), high potassium levels in the blood (hyperkalemia), sudden or worsening kidney problems, and Addison’s disease.
Spironolactone works in the far part of the kidney tubules, where it blocks the action of a hormone called aldosterone. This causes the body to get rid of extra salt and water, but it helps the body keep hold of potassium and hydrogen ions.
Adult: PO Oedema Initial: 100 mg/day, up to 400 mg/day based on response.
Child: Neonates: 1-2 mg/kg daily; 1 mth-12 yr: 1-3 mg/kg daily; 12-18 yr: 50-100 mg daily. To be given in 1-2 divided doses.
Elderly: Initially, 25-50 mg/day in 1-2 divided doses, may increase by 25-50 mg every 5 days when necessary.

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
