Precautions and warnings

Patients who are taking diuretics or have low sodium levels should stop taking the diuretics or increase their sodium intake before starting treatment. If you have kidney problems, lupus (SLE), other autoimmune diseases, or are taking medicines that suppress the immune system or lower white blood cells, your white blood cell count and urine protein should be checked before and during treatment. Use with caution if you are breastfeeding, have porphyria, or severe heart failure.

Drug Interactions

Taking NSAIDs at the same time can make this medicine less effective at lowering blood pressure and increase the chance of kidney problems. Taking potassium supplements, potassium-sparing diuretics, or other drugs like heparin can raise the risk of having too much potassium in your blood. The risk of low white blood cell count is higher if used with drugs like procainamide, allopurinol, some cancer drugs, or immune-suppressing medications. This medicine can also raise the risk of lithium toxicity. Using gold injections (sodium aurothiomalate) may increase the chance of nitritoid reactions such as flushing or low blood pressure.

Serious Warning: Combining this medication with aliskiren in people with diabetes can dangerously increase the risk of low blood pressure, high potassium levels, and kidney problems—including sudden kidney failure.

Contraindications

Do not use this medication if you are allergic to it. Avoid if you have narrowing of both renal arteries, hereditary angioedema, kidney problems, or if you are pregnant.

Mode of actions

Captopril works by blocking the conversion of angiotensin I (ATI) into angiotensin II (ATII). As a result, there is less ATII and less aldosterone produced. It also raises the levels of plasma renin activity and bradykinin. With lower ATII, the body holds onto less sodium and water, which helps relax blood vessels and lowers blood pressure.

Dosage & Administration

Adult: PO HTN Initial: 12.5 mg twice daily. Maintenance: 25-50 mg twice daily. Max: 50 mg 3 times/day.

Heart failure Initial: 6.25-12.5 mg 2-3 times/day. Max: 50 mg 3 times/day.

Post MI Start 3 days after MI. Initial: 6.25 mg/day, may increase after several wk to 150 mg/day in divided doses if needed and tolerated.

Diabetic nephropathy 25 mg 3 times/day.

Renal impairment:
CrCl (ml/min)

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
