Precautions and warnings
Vertigo and tinnitus are symptoms of renal failure. If you see signs of ototoxicity, neurotoxicity, or hypersensitivity, stop taking it; if you're lactating, stop using it. The treatment period of andgt;14 days has not been proven to be safe. Before and during therapy, keep an eye on your kidney function.
Side-effects
- Tinnitus
- vertigo; ataxia
- overt deafness. Potentially Fatal: Ototoxicity
- nephrotoxicity
- neuromuscular blockade.
Drug Interactions
When taken concurrently, amphotericin B may cause nephrotoxicity and limit amikacin clearance.
Possibly lethal: Other nephrotoxic or ototoxic medicines have increased ototoxic or nephrotoxic effects. Neuromuscular blocking medications improve neuromuscular blockade. Potent diuretics increase the risk of ototoxicity.
Contraindications
Pregnancy, perforated ear drum, myasthenia gravis, hypersensitivity.
Mode of actions
Amikacin binds to 30S ribosomal subunits of susceptible bacteria, thus inhibiting its protein synthesis.
Dosage & Administration
Parenteral
Uncomplicated urinary tract infections
Adult: 250 mg bid, given via IM, IV inj over 2-3 minutes or as IV infusion.
Severe Gram-negative infections resistant to gentamicin and tobramycin
Adult: 15 mg/kg daily in equally divided doses injected every 8 or 12 hr for 7-10 days. Max: Up to 500 mg every 8 hr in life-threatening infections. Max cumulative dose: 15 g. Doses may be given via IM, slow IV inj over 2-3 minutes or IV infusion.
Child: 15 mg/kg daily in equally divided doses injected every 8 or 12 hr for 7-10 days. Neonates: 10 mg/kg daily in 2 divided doses.
Impaired renal function: In patient with impaired renal function the daily dose should be reduced and/or the intervals between doses increased to avoid accumulation of the drug. Simple doses schedule for renal impairment is given below:
Renal function Dosage schedule
Mild impairment 500 mg every 18 hours
Moderate impairment 500 mg every 24 hours
Severe impairment 250 mg every 24 hours.
Pregnancy & Lactation
This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.
There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
