Precautions and warnings

If pregnancy is detected, stop using Amlodipine+Benazepril as soon as feasible. Caution is advised in the elderly, those with renal or hepatic impairment, those with congestive heart failure, those with severe aortic stenosis, those with unilateral or bilateral renal artery stenosis, and those who are undergoing surgery or anaesthetic. Symptomatic hypotension can be caused by the combination of amlodipine and benazepril, especially in individuals with salt/volume depletion. ACE inhibitors (ACEi) have been linked to cholestatic jaundice and fulminant hepatic necrosis; if the patient develops jaundice or a substantial increase in liver enzymes, discontinue the medication. Renal function and potassium levels in the blood should be checked on a regular basis. In ACEi-treated individuals, angioedema of the face, extremities, lips, tongue, glottis, larynx, and intestinal angioedema have been described. As with all ACE inhibitors, it can produce a chronic dry cough. The drug's safety and efficacy in children have yet to be determined.

Drug Interactions

When used with catecholamine-depleting medicines, there is a risk of hypotension and/or significant bradycardia. To avoid rebound hypertension, clonidine should be terminated a few days after the beta-blocker has been stopped; if replacing clonidine with beta-blocker, beta-blocker should be started only after clonidine has been stopped for several days. Concurrent use with prostaglandin synthase inhibitors (such as indomethacin) may diminish beta-blocker hypotensive effects.

Contraindications

Patients having a history of angioedema, as well as pregnant and lactating women.

Mode of actions

Amlodipine is a calcium antagonist that is a dihydropyridine. Through suppression of transmembrane Ca ion influx into vascular smooth muscle and cardiac muscle, it decreases blood pressure by relaxing coronary vascular smooth muscles and causing coronary vasodilation. It's also a vasodilator for the peripheral arteries. Benazepril and its active metabolite benazeprilat block ACE, resulting in lower plasma angiotensin II levels and, as a result, lower aldosterone secretion.

Dosage & Administration

Adult: PO Per combination cap contains Amlodipine 2.5-10 mg and benazepril 10-40 mg: Amlodipine component: 2.5-10 mg once daily; benazepril component: 10-40 mg once daily.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Another study shows that this medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
