When taken in the second and third trimesters of pregnancy, drugs that affect the renin-angiotensin system can induce fetal damage and death. Once a pregnancy is detected, olmesartan medoxomil should be stopped as soon as feasible. Patients who are deficient in volume and/or salt may experience symptomatic hypotension. Oliguria, progressive azotemia, and acute renal failure have all been linked to this condition (rare). Severe hypotension and renal insufficiency are more likely in patients with bilateral or unilateral renal artery stenosis. Caution is advised in the case of renal impairment; serum creatinine and potassium levels should be checked on a regular basis. Use with caution if you have severe renal impairment (CrClandlt; 20 mL/min) or severe hepatic impairment (CrClandlt; 20 mL/min). Patients with aortic or mitral valve stenosis or obstructive hypertrophic cardiomyopathy should be cautious.
Angiotensin receptor antagonists may not work in patients with primary aldosteronism.
Lactation is not advised.
When used with catecholamine-depleting medicines, there is a risk of hypotension and/or significant bradycardia. To avoid rebound hypertension, clonidine should be terminated a few days after the beta-blocker has been stopped; if replacing clonidine with beta-blocker, beta-blocker should be started only after clonidine has been stopped for several days. Concurrent use with prostaglandin synthase inhibitors (such as indomethacin) may diminish beta-blocker hypotensive effects. ACE inhibitors, K-sparing diuretics, K salts or K supplements, and medications that may increase serum K all increase the risk of hyperkalemia (for example, ciclosporin, eplerenone). Along with other antihypertensives, it may enhance the effects of decreasing blood pressure. When used with NSAIDs, glomerular filtration might be reduced, resulting in acute renal failure. Lithium toxicity and serum concentrations may be increased.

Biliary blockage during pregnancy (second and third trimesters).
Amlodipine relaxes the smooth muscle of the peripheral and coronary arteries. During depolarization, it inhibits Ca ion entry into the slow channels or select voltage-sensitive channels of the vascular smooth muscle and heart, resulting in coronary vasodilation. In patients with vasospastic angina, it also improves myocardial oxygen delivery.
Olmesartan is a selective and competitive angiotensin II Type 1 (AT1) receptor antagonist that prevents angiotensin II from acting as a vasoconstrictor and secreting aldosterone. Olmesartan relaxes blood arteries, decreasing blood pressure and increasing blood supply and oxygen to the heart as a result.
Adult: Initial: 10-20 mg once daily. Antihypertensive effect is substantially present within 2 wk of therapy initiation. Dose may be increased up to 40 mg once daily if needed. May consider a lower starting dose in patients with possible depletion of intravascular volume (e.g. patients receiving concomitant diuretics, especially those with renal impairment).
Child: 6 to 16 yr: 20-35 kg: 10 mg once daily, may increase to maximum of 20 mg once daily. >35 kg: 20 mg once daily, may increase to maximum of 40 mg once daily.
Elderly: May consider lower starting dose of 5-10 mg daily.
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Another study shows that this medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.
There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
