Precautions and warnings
Methyldopa should be used carefully in people who have had liver problems before. Some patients taking methyldopa may notice swelling (edema) or weight gain, but this can sometimes be managed with a water pill (diuretic). However, if the swelling gets worse or if signs of heart failure appear, methyldopa should be stopped. Sometimes, high blood pressure returns after dialysis because methyldopa is removed from the body during this process. Rarely, people with serious brain blood vessel disease may develop involuntary movements while taking methyldopa. If this happens, treatment should be stopped.

- significant adverse effects due to Methyldopa have been infrequent
- this agent usually is well tolerated.
Drug Interactions
Some drugs can reduce the blood pressure-lowering effect, such as phenothiazines, TCAs, and possibly amphetamines. Taking this with levodopa can make low blood pressure worse and may also cause psychosis. If you take iron by mouth, the medicine may not work as well because less of it gets absorbed. You may need less general anaesthetic when using this medicine. Ephedrine may not work as well.
**Warning:** Combining this medicine with MAOIs can cause dangerous high blood pressure. The risk of lithium toxicity is also higher.
Contraindications
Methyldopa should not be used in patients who:
- Have active liver problems, such as hepatitis or cirrhosis.
- Have had liver issues before that were linked to Methyldopa treatment.
- Are allergic to any ingredient in Methyldopa.
- Are currently taking monoamine oxidase (MAO) inhibitors.
Mode of actions
Methyldopa activates alpha-2 adrenoceptors, which decreases the activity of the sympathetic nervous system and lowers blood pressure.
Dosage & Administration
Oral
Hypertension
Adult: Monotherapy: Initially, 250 mg bid-tid for 2 days; adjust according to response not more than every 2 days. Maintenance: 0.5-2 g daily. Max: 3 g daily. Combination therapy: Initial dose should not exceed 500 mg daily in divided doses.
Child: Initially, 10 mg/kg or 300 mg/m2 daily in 2-4 divided doses; increase as necessary. Max: 65 mg/kg, 2 g/m2 or 3 g daily, whichever is least.
Elderly: Initially, 125 mg bid; gradually increase according to response. Max: 2 g daily.
Pregnancy & Lactation
Although animal reproduction studies have shown an adverse effect even sometimes unable to demonstrate the risk on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required.

There is no specific information about whether this medicine is passed on to the baby through breast milk. Discuss with your doctor before breastfeeding.
