Precautions and warnings
اپنے ڈاکٹر کو بتائیں کہ اگر آپ کو درج ذیل طبی حالتوں میں سے کوئی ہے: گردے کی بیماری، جگر کی بیماری، خون کی کچھ خرابیاں، ٹرائیمتھوپریم یا سلفا ادویات کی وجہ سے خون کی خرابی کی تاریخ، وٹامن کی کمی (فولیٹ یا فولک ایسڈ)، شدید الرجی، دمہ، ہڈیوں کے گودے کے کام میں کمی، میٹابولک عارضہ جیسے کہ پوٹاشیم کی اعلی سطح یا خون میں سوجن کی کم سطح۔
یہ دوا زندہ بیکٹیریل ویکسین جیسے ٹائیفائیڈ ویکسین کی تاثیر کو خراب کر سکتی ہے۔ کوئی ویکسین یا ٹیکے لگوانے سے پہلے اپنے ڈاکٹر کو بتائیں کہ آپ یہ دوا لے رہے ہیں۔
Side-effects
- گردوں کی ناکامی، متلی، الٹی، اسہال، کشودا؛ جلد پر دھبے، چھپاکی۔ ممکنہ طور پر مہلک: سٹیونز جانسن سنڈروم، ایگرانولو سائیٹوسس، زہریلا ایپیڈرمل نیکرولیسس، ہیپاٹک نیکروسس۔
Drug Interactions
"خون کو پتلا کرنے والے" (جیسے وارفرین)، ڈوفیٹیلائیڈ، میتھینامین، اور میتھوٹریکسیٹ کچھ ایسی مصنوعات ہیں جو اس دوا کے ساتھ تعامل کر سکتی ہیں۔
پیریمیتھامین کے ساتھ شریک منتظم میگالوبلاسٹک انیمیا کا باعث بن سکتا ہے، جو مہلک ہو سکتا ہے۔ Cyclosporin گردے کو نقصان پہنچانے کا سبب بنتا ہے۔
Contraindications
مریض کو اس سے پرہیز کرنا چاہئے اگر اس میں فعال اجزاء کے لئے انتہائی حساسیت ہے، شدید گردوں یا جگر کی کمی ہے۔
Mode of actions
co-trimoxazole میں اس کے اجزاء (sulfamethoxazole اور trimethoprim) کی ہم آہنگی کی سرگرمیوں کو دس سے ضرب دیا جاتا ہے۔ سلفامیتھوکسازول PABA سے ڈائی ہائیڈرو فولک ایسڈ کی تخلیق کو روکتا ہے، بیکٹیریل فولک ایسڈ کی ترکیب اور نشوونما کو روکتا ہے۔ Trimethoprim فولک ایسڈ کے راستے کے خامروں کو روکتا ہے، ڈائی ہائیڈرولک ایسڈ کو ٹیٹراہائیڈروفولیٹ میں تبدیل ہونے سے روکتا ہے۔ ای کولی، کلیبسیلا ایس پی پی، اینٹروبیکٹر ایس پی پی، ایم مورگنی، پی میرابیلیس، پی ولگارس، ایچ انفلوئنزا، اسٹریپ نمونیا، نیوموسسٹس (کیرینی) جیروویسی، اور سائکلوسپورا ایس پی پی۔ سب کو-ٹرائیموکسازول کے لیے حساس ہیں۔
Dosage & Administration
بالغ: پی او ٹیبلٹ: 2 گولی دن میں دو بار۔ شدید حالتوں میں، دن میں 3 بار۔ ڈی ایس ٹیبلٹ: 1 گولی دن میں دو بار۔
معطلی: 1-2 چائے کا چمچ دن میں دو بار۔
GI انفیکشن؛ سانس اور پیشاب کی نالی کے انفیکشن؛ حساس انفیکشن 960 ملی گرام روزانہ دو بار، شدید صورتوں کے لیے 2.88 جی فی دن تک 2 تقسیم شدہ خوراکوں میں۔
نیوموسسٹس (کیرینی) جیروویسی نمونیا 14-21 دنوں کے لیے 2-4 تقسیم شدہ خوراکوں میں 120 mg/kg/day تک۔ ایڈز کے مریضوں میں انفیکشن پروفیلیکسس 960 ملی گرام فی دن۔
بچہ: 6 ہفتہ-5 ماہ: 120 ملی گرام بولی؛ 6 ماہ-5 سال: 240 ملی گرام بولی؛ 6-12 سال: 480 ملی گرام بولی۔
گردوں کی خرابی: بالغوں اور بچوں کے لیے خوراک میں کمی> 12 سال:
CrCl (ml/min)
15-30 آدھی معیاری خوراک۔
Pregnancy & Lactation
حمل کے دوران اس دوا کی اجازت نہیں ہے۔ اگر آپ حاملہ ہیں تو اسے استعمال نہ کریں۔ یہ جلد اور پھیپھڑوں کے ذریعے جذب ہو سکتا ہے جس کی وجہ سے آپ کے بچے کے اعضاء کو نقصان پہنچ سکتا ہے اور پیدائشی معذوری کا سبب بن سکتا ہے۔ علاج کے دوران اور اس دوا کو روکنے کے بعد چھ ماہ تک سخت موثر برتھ کنٹرول۔ اگر آپ حاملہ ہو جاتی ہیں، تو اس دوا کو لینا بند کر دیں اور فوراً اپنے ڈاکٹر سے بات کریں۔
اس بارے میں کوئی خاص معلومات نہیں ہے کہ آیا یہ دوا چھاتی کے دودھ میں جاتی ہے یا نہیں۔ بچوں کے لیے ممکنہ خطرات کو مدنظر رکھتے ہوئے، اس دوا کے استعمال کے دوران دودھ پلانے کی سفارش نہیں کی جاتی ہے اور آخری خوراک کے کم از کم دو ہفتے بعد۔ دودھ پلانے سے پہلے اپنے ڈاکٹر سے مشورہ کریں۔
