Precautions and warnings

This medicine may sometimes cause unexpected tightening of the airways (paradoxical bronchospasm). Use carefully in people who are sensitive to drugs that stimulate the nervous system (sympathomimetic agents).

Side-effects

  • Adverse Effects: In common with other beta-agonists containing products
  • side effects of Windel Plus can include fine tremor of skeletal muscles
  • nervousness
  • less frequently
  • tachycardia
  • dizziness
  • palpitations or headache
  • especially in hypersensitive patients. Potentially serious hypokalaemia may result from prolonged
  • / or high dose beta2-agonist therapy. As with use of other inhalation therapy
  • cough
  • local irritation and
  • less commonly
  • inhalation induced bronchospasm can occur. As with other beta-mimetics
  • nausea
  • vomiting
  • sweating
  • weakness
  • myalgia/muscle cramps may occur. In rare cases decrease in diastolic blood pressure
  • increase in systolic blood pressure
  • arrhythmias
  • particularly after higher doses
  • may occur. In individual cases psychological alterations have been reported under inhalation therapy with beta-mimetics. The most frequent non-respiratory anticholinergic related adverse events are dryness of mouth
  • dysphonia. There have been isolated reports of ocular complications (i.e. mydriasis
  • increased intraocular pressure
  • angle-closure glaucoma
  • and eye pain) when aerosolised ipratropium bromide either alone or in combination with adrenergic beta2-agonist
  • has escaped into the eyes. Ocular side effects
  • gastrointestinal motility disturbances
  • urinary retention may occur in rare cases
  • are reversible.

Drug Interactions

Using this medicine with psychotropic drugs can increase anticholinergic side effects. It may also make propranolol less effective. When taken with MAOIs, tricyclic antidepressants, or amphetamines, it can increase heart-related side effects like a fast heartbeat and palpitations.

Contraindications

The combination of Salbutamol and Ipratropium Bromide should not be used in people who have hypertrophic obstructive cardiomyopathy, tachyarrhythmia, or who are allergic to atropine, its related drugs, or any other ingredient in the medicine.

Mode of actions

Ipratropium bromide is a type of medicine called an anticholinergic. It works by blocking acetylcholine, a chemical in the body, from causing certain reflexes in the lungs. By doing this, it stops the increase of a substance called cyclic GMP inside the cells of the airway muscles, which would otherwise make the muscles tighten.

Salbutamol is a medicine that directly activates beta-2 receptors in the airways. It relaxes the airway muscles, which helps to open up the airways and makes breathing easier.

Dosage & Administration

Adults (including elderly): Per 3 mL nebulising soln contains ipratropium bromide 0.5 mg and salbutamol base 2.5 mg: Initial: 3 mL 6 hrly. Max: 3 mL 4 hrly.

One ampoule as required for the relief of symptoms or as directed. Up to three to four ampoules daily.

Patients should be advised to consult a doctor or the nearest hospital immediately in the case of acute or rapidly worsening dyspnoea if additional inhalations do not produce an adequate improvement.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
