Precautions and warnings

Kidney problems: Use streptomycin with caution in patients with kidney issues. Do not use streptomycin creams or inhalers if breastfeeding. If treatment will last a long time, have hearing tests regularly. Check kidney function often. Patients with kidney problems have a higher chance of side effects related to the nervous system (neurotoxicity). Neurotoxicity can cause breathing problems, especially if streptomycin is given right after anesthesia or muscle relaxants.

Drug Interactions

H1-receptor blockers can hide the early symptoms of ear toxicity (ototoxicity). They may slow down how quickly the body gets rid of zalcitabine and could block the activity of ?-galactosidase.

Serious Warning: Using this drug with other aminoglycosides, vancomycin, or some cephalosporins can increase the risk of kidney damage. It can also make ear toxicity worse if taken with other aminoglycosides, ethacrynic acid, mannitol, furosemide, or other diuretics.

Mode of actions

Streptomycin is a type of antibiotic that kills bacteria by stopping them from making proteins. It works against several types of bacteria, including the ones that cause tuberculosis (Mycobacterium tuberculosis), plague (Pasteurella pestis), and tularemia (P. tularensis). It is also effective against Brucella, Haemophilus influenzae, Haemophilus ducreyi, Klebsiella pneumoniae, Escherichia coli (E. coli), Proteus species, Aerobacter aerogenes, Streptococcus faecalis, and Streptococcus viridans.

Dosage & Administration

Adult: IM Tuberculosis 15 mg/kg/day. Max: 1 g/day. For intermittent therapy: 25-30 mg/kg/day 2-3 times/wk. Max: 1.5 g/dose.
Child: 20-40 mg/kg (max: 1 g) daily or 25-30 mg/kg (max: 1.5 g) 2-3 times wkly.
Elderly: >60 kg: Dosage reduction is required.

Mycobacterium avium complex infections As adjunct w/ macrolide, rifamycin and ethambutol: 15 mg/kg 3 times/wk for the 1st 2-3 mth for severe disease.

Bacterial endocarditis W/ penicillin: Streptococcal endocarditis: 1 g twice daily for the 1st wk, 500 mg twice daily for the 2nd wk; Enterococcal endocarditis: 1 g twice daily for 2 wk, then 500 mg twice daily for 4 wk. Terminate therapy if ototoxicity occurs.

Child: To be used concurrently with penicillin. Enterococcal endocarditis: 20-30 mg/kg daily given in 2 divided doses. Recommended treatment duration: 4-6 wk for patients native valve endocarditis and at least 6 wk for patients with prosthetic valve or other prosthetic cardiac material.

Brucellosis 1 g/day for 14-21 days, w/ oral doxycyline for 6 wk. Plague 2 g daily for at least 10 days. Tularaemia 1-2 g/day in divided doses for 7-14 days and until the patient is afebrile for 5-7 days.

Child: 15 mg/kg bid (max: 2 g daily) for at least 10-14 days.

Renal impairment: Modification in dose or dosing interval may be required.
CrCl (ml/min)
10-50 Dosing interval: Every 24-72 hr.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
