Hepatic or renal disease, a pregnant woman, a mother who breastfeeds her baby, hypothyroidism, adrenocortical insufficiency, asthma, prostatic hyperplasia, shock, hypotension, inflammatory or obstructive bowel disorders, myasthenia gravis, infants/neonates, hypothyroidism, adrenocortical insufficiency, asthma, prostatic hyperplasia, Patients who are elderly or handicapped should take a lower dose. In narcotic users, it may exacerbate withdrawal symptoms.
When used with medicines that increase or inhibit the cytochrome P450 isoenzyme CYP3A4, plasma-buprenorphine concentrations may be altered. Other CNS depressants, such as alcohol, anesthetics, anxiolytics, hypnotics, TCAs, and antipsychotics, have enhanced depressant effects.
Diazepam can cause respiratory and cardiac collapse, which can be fatal.
Patients on established opioid agonists, acute drunkenness, convulsive disorders, head injuries, elevated intracranial pressure, comatose patients, resp depression, and obstructive airway disease.
Buprenorphine works as an analgesic by attaching to mu-opioid receptors in the central nervous system. Its analgesic effect lasts longer than that of morphine. It has a minimal level of physical dependence due to its partial agonist action. Buprenorphine and morphine have similar dose-dependent effects on the respiratory system.
Adult: Sublingual Moderate to severe pain 200-400 mcg 6-8 hrly. IV Perioperative analgesia 300-450 mcg via slow inj. IV/IM Moderate to severe pain 300-600 mcg 6-8 hrly. IM Anesth premed 300 mcg.
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
