Precautions and warnings
肝或腎疾病、孕婦、哺乳嬰兒的母親、甲狀腺功能低下、腎上腺皮質功能不全、氣喘、攝護腺增生、休克、低血壓、發炎或阻塞性腸道疾病、重症肌無力、嬰兒/新生兒、甲狀腺功能低下、腎上腺皮質功能不全、氣喘、攝護腺增生、老年人或殘障者應減少劑量。 對於麻醉劑使用者來說，它可能會加劇戒斷症狀。
Side-effects
- 鎮靜、噁心、頭暈、眩暈、低血壓、瞳孔縮小、頭痛、通氣不足、呼吸或中樞神經系統抑制；心跳過速、心搏過緩、尿瀦留、昏迷、嘔吐、嗜睡、出汗、精神錯亂、口乾、出汗、戒斷症候群。
Drug Interactions
當與增加或抑制細胞色素 P450 同功酶 CYP3A4 的藥物一起使用時，血漿丁丙諾啡濃度可能會改變。 其他中樞神經系統抑制劑，如酒精、麻醉劑、抗焦慮藥、安眠藥、TCA 和抗精神病藥，具有增強的抑製作用。
地西泮可導致呼吸和心臟衰竭，這可能是致命的。
Contraindications
服用鴉片類激動劑、急性醉酒、驚厥、頭部受傷、顱內壓升高、昏迷、呼吸抑制和阻塞性氣道疾病的患者。
Mode of actions
丁丙諾啡透過附著在中樞神經系統中的 mu-阿片受體上發揮鎮痛劑的作用。 其鎮痛作用比嗎啡持續時間更長。 由於其部分激動劑作用，它具有最低程度的身體依賴性。 丁丙諾啡和嗎啡對呼吸系統有類似的劑量依賴性作用。
Dosage & Administration
成人：舌下含服中度至重度疼痛 200-400 微克，每 6-8 小時一次。 IV 圍手術期鎮痛 300-450 mcg，緩慢注射。 IV/IM 中度至重度疼痛 300-600 微克，每 6-8 小時一次。 IM 麻醉預藥 300 微克。
Pregnancy & Lactation
儘管動物生殖研究顯示對胎兒有不良影響，並且在孕婦中沒有充分且良好對照的研究，但在懷孕期間，儘管存在潛在風險，但在確實需要時可以使用該藥物。
在懷孕最後 3 個月內使用這種藥物的新生兒母親可能很少會出現困倦、肌肉僵硬或顫抖、餵食或呼吸困難、持續哭泣等症狀。 如果您發現寶寶有任何這些症狀，尤其是在第一個月內，請立即告訴醫生。
