The patient is experiencing shock caused by a heart attack and also has a history of peripheral vascular disease. Before giving an infusion, make sure to correct any low blood volume. Be cautious if the patient has a history of blocked blood vessels, such as atherosclerosis, Raynaud's disease, Buerger's disease, or diabetic endarteritis, as these conditions may cause an uneven rise in diastolic blood pressure. Take extra care if the patient is pregnant.
Cyclopropane and some anesthetics containing halogens can make the heart more sensitive to dopamine, which may cause dangerous heart rhythms. MAO inhibitors make the effects of dopamine last longer and become stronger. Ergot drugs increase dopamine's ability to tighten blood vessels. Alpha-blockers reveal dopamine's effects on beta receptors.
Pheochromocytoma, uncorrected tachyarrhythmias, ventricular fibrillation. Hypersensitivity.
At low doses, dopamine activates dopaminergic receptors, which causes blood vessels in the kidneys and digestive organs to widen. At higher doses, it also stimulates both dopaminergic and beta-adrenergic receptors, leading to increased heart rate, stronger heart contractions, and continued widening of blood vessels in the kidneys. When given at low infusion rates, dopamine causes blood vessels in the kidneys, digestive organs, heart, and brain to widen. When the dose is higher, it causes blood vessels in the muscles to narrow and blood pressure to increase.
Adult: IV Initial: 1-5 mcg/kg/min, up to 5-10 mcg/kg/min according to patient's response. For seriously ill patients: up to 20-50 mcg/kg/min may be used.
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
