Precautions and warnings

Do not use this medication if you have ever had an allergic reaction to aminoglycosides, are pregnant, have liver problems, or have a perforated (broken) eardrum.

Side-effects

  • Dizziness or vertigo; acute renal failure
  • interstitial nephritis
  • acute tubular necrosis; electrolyte imbalances; transient elevation of serum bilirubin
  • aminotransferases; purpura; nausea
  • vomiting; convulsions
  • mental depression
  • hallucinations. Atrophy or rat necrosis at inj sites. Potentially Fatal: Nephrotoxicity
  • ototoxicity
  • neuromuscular blockade (may unmask or aggravate myasthaenia gravis).

Drug Interactions

This medicine works well with ampicillin, benzylpenicillin, and other beta-lactam antibiotics. Using it together with anesthetics or opioids can increase the risk of severe breathing problems. It can also lower how much zalcitabine is removed by the kidneys and may cause low calcium levels if taken with bisphosphonates. Do not use this drug with agalsidase alfa or beta, as it may block the action of ?-galactosidase.

Serious Warnings: There is a higher risk of ear damage (ototoxicity) if taken with ethacrynic acid or furosemide. The risk of kidney damage (nephrotoxicity) is increased when combined with cephalosporins, ciclosporin, cisplatin, vancomycin, hydrocortisone, or indometacin. This drug can also make muscle-relaxing drugs (neuromuscular blockers) work more strongly.

Contraindications

Do not use if you have a history of allergy to aminoglycosides, are pregnant, have liver problems, or have a ruptured eardrum.

Mode of actions

Gentamicin is an aminoglycoside antibiotic that attaches to the 30s and 50s parts of a bacteria’s ribosome. This stops the bacteria from making proteins properly, which damages their cell membrane and helps kill the bacteria.

Dosage & Administration

Adult: IM Susceptible infections 3-5 mg/kg/day in divided doses 8 hrly for 7-10 days. Surgical prophylaxis 120 mg before induction of anesth, w/ penicillin, vancomycin or teicoplanin.

Pediatric
Up to 2 weeks: 3 mg/kg/every 12 hours
2 weeks to 12 years: 2 mg/kg/every 8 hours or as directed by the physician.

Renal impairment: Haemodialysis: Administer dose after dialysis session and monitor levels.

>60 Administer every 8 hr.
40-60 Administer every 12 hr.
20-40 Administer every 24 hr.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
