Precautions and warnings
- High pressure inside the skull (raised intracranial pressure)
- Low blood pressure (hypotension)
- Low blood volume (hypovolaemia)
- Mitral valve prolapse (a heart valve problem)
- Low oxygen in the blood (arterial hypoxaemia)
- Glaucoma (an eye condition with high pressure)
- Older age (elderly)
- Low thyroid activity (hypothyroidism)
- Poor nutrition (malnutrition)
- Pregnancy
- Breastfeeding (lactation)
Side-effects

- vertigo. Potentially Fatal: Severe hypotension
- circulatory collapse.
Drug Interactions
Taking this medication with alcohol or drugs that widen blood vessels (vasodilators) can increase the risk of low blood pressure. Using it with calcium channel blockers can cause severe drops in blood pressure when standing up (orthostatic hypotension). Dihydroergotamine may reduce its blood vessel-widening effects, while ergotamine’s effects may become stronger. If you use the sublingual form with disopyramide, it may not work as well.
**Danger:** Combining this medication with phosphodiesterase-5 inhibitors (like those for erectile dysfunction) can cause dangerously low blood pressure.
Contraindications
Very low blood pressure or anaemia, low blood volume, heart failure caused by a blockage, or increased pressure in the brain due to head injury or brain bleeding.
Mode of actions
Isosorbide dinitrate helps relax the smooth muscles in blood vessels by increasing the amount of cyclic-GMP. This relaxation lowers the pressure in the left side of the heart before it pumps (preload) and also reduces the resistance in the arteries (afterload).
Dosage & Administration
Adult: PO Long term management of angina 20-120 mg/day in divided doses. Max: 240 mg/day. Heart failure 30-160 mg/day in divided doses. Max: 240 mg/day.
Sublingual Heart failure 5-15 mg 2-3 hrly. Acute angina As tab: 2.5-10 mg. As spray: 1-3 sprays (1.25 mg/spray).
Pregnancy & Lactation
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
