Precautions and warnings

Older adult with a history of severe allergic reactions (anaphylaxis) to different allergens. Has other medical conditions, including first-degree heart block, poor blood flow in the limbs (peripheral arterial disease), diabetes, stable heart failure, myasthenia gravis, mental health issues, and reduced kidney function. Note: May hide symptoms of overactive thyroid (like fast heartbeat).

Drug Interactions

Plasma levels can rise when this drug is taken with strong CYP2D6 inhibitors such as paroxetine, fluoxetine, propafenone, thioridazine, or quinidine. Using this medication together with certain antiarrhythmic drugs (like amiodarone or disopyramide) or nondihydropyridine calcium channel blockers (such as diltiazem or verapamil) may cause heart conduction problems. There is a higher risk of slowed heart rate or conduction issues if combined with other beta-blockers or digoxin. Taking this drug with drugs that reduce catecholamines (like reserpine) may also increase the risk of low blood pressure or a slow heart rate. Suddenly stopping clonidine can raise the risk of rebound high blood pressure.

Contraindications

People with liver problems, sick sinus syndrome, or 2nd/3rd-degree heart block (unless they have a pacemaker) should not use this. It is also not suitable for those with a history of asthma, metabolic acidosis, severe poor blood flow to the limbs, very slow heartbeat, cardiogenic shock, severe heart failure, or untreated phaeochromocytoma. This medicine should also be avoided during pregnancy and breastfeeding.

Mode of actions

Nebivolol mainly targets beta-1 adrenergic receptors and helps widen blood vessels by directly acting on the blood vessel lining, which leads to the release of nitric oxide. It does not have its own stimulating or membrane-stabilizing effects.

Dosage & Administration

Adult: PO HTN 5 mg/day. Max: 40 mg once daily. Heart failure Initial: 1.25 mg once daily. May double dose 1-2 wkly up to a max of 10 mg once daily.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
