Precautions and warnings

Be cautious in people with low blood volume, narrowing of the kidney arteries (renal artery stenosis), severe heart failure, kidney problems, or mild to moderate liver issues. It is important to monitor blood potassium levels regularly. Extra care is also needed for older adults.

Side-effects

  • Dizziness; headache; dose-related orthostatic hypotension; rash; angioedema; hyperkalaemia; myalgia; resp tract disorders; back pain; GI disturbances; fatigue; increase in BUN
  • serum creatinine; abdominal pain; dry cough; LFT elevations. Potentially Fatal: Blood dyscrasias (e.g. neutropenia).

Drug Interactions

NSAIDs can reduce the blood pressure-lowering effects and may increase the risk of kidney problems. Using potassium-sparing diuretics, potassium supplements, or salt substitutes that contain potassium raises the risk of high potassium levels in the blood.
Potentially serious: Combining this medicine with aliskiren in people with diabetes or kidney problems (low GFR) can cause dangerously low blood pressure, high potassium, and kidney issues, including acute kidney failure.

Contraindications

Do not use if you are allergic to this medicine, have serious liver problems (such as severe liver damage, cirrhosis, or blocked bile ducts), or have a condition called primary hyperaldosteronism. Also, avoid during the second and third trimesters of pregnancy and while breastfeeding.

Mode of actions

Valsartan is a medicine that lowers blood pressure by blocking a receptor called angiotensin II type 1 (AT1). By doing this, it stops angiotensin II from causing blood vessels to tighten, from triggering the release of aldosterone (a hormone), and from making the kidneys reabsorb sodium.

Dosage & Administration

Adult: PO HTN Initial: 80 mg once daily, up to 160 mg once daily. Max: 320 mg/day.
Heart failure 40 mg twice daily, up to 160 mg twice daily if tolerated.
Post-MI Initial: 20 mg twice daily, up to 160 mg bid if tolerated.

Elderly: No dosage adjustment needed.
Hepatic impairment: Mild to moderate: Max: 80 mg once daily. Severe: Contraindicated.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
