Hypotension, poor cardiac reserve, heart failure. Pregnancy.
Side-effects
- Dizziness; flushing; headache; hypotension; peripheral oedema; tachycardia; palpitations; GI disturbances; increased micturition frequency; lethargy; eye pain; depression; ischaemic chest pain; cerebral or myocardial ischaemia; transient blindness; rashes; fever; abnormal liver function; gingival hyperplasia; myalgia; tremor; impotence; dyspnea.
Drug Interactions
Taking isradipine at the same time as enzyme-inducing drugs (such as rifampicin, phenobarbital, or carbamazepine) can lower the amount of isradipine in your blood. Cimetidine may increase how much isradipine your body absorbs. Drugs that inhibit the CYP3A4 enzyme (like macrolide antibiotics, HIV protease inhibitors, azole antifungals, or delavirdine) may also raise isradipine blood levels.
Contraindications
Do not use if you have: heart failure due to poor heart function (cardiogenic shock), a recent heart attack (MI) or sudden worsening chest pain (unstable angina), severe narrowing of the aortic heart valve (aortic stenosis), are breastfeeding (lactation), or have porphyria.
Mode of actions
Isradipine is a type of calcium channel blocker from the dihydropyridine group. It works by stopping calcium ions from entering specific areas of the blood vessels and heart muscle during their active phase. This action relaxes the muscles in the walls of the heart’s blood vessels, leading to wider (dilated) coronary arteries and improved blood flow.
Dosage & Administration
Oral
Hypertension
Adult: Initially, 2.5 mg bid, increase if necessary after 3-4 wk to 5 mg bid, or 10 mg bid as required.
Elderly: Initially, 1.25 mg bid. Maintenance: 2.5 or 5 mg once or twice daily.
Hepatic impairment: Initially 1.25 mg bid titrated slowly to maintenance: 2.5 or 5 mg once or twice daily.
Pregnancy & Lactation
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
