Precautions and warnings

Check serum potassium levels before starting treatment, during the first week, and regularly after that. The safety and effectiveness of this treatment have not been proven in patients with severe liver problems or in children.

Drug Interactions

Taking Eplerenone with CYP3A4 inhibitors (like ketoconazole, voriconazole, itraconazole, fluconazole, clarithromycin, diltiazem, saquinavir, verapamil, or ritonavir) may increase the amount of Eplerenone in your blood. On the other hand, CYP3A4 inducers (such as rifampicin, carbamazepine, phenytoin, or phenobarbital) can make Eplerenone less effective.

Eplerenone can increase the risk of high potassium levels when taken with ACE inhibitors, angiotensin II receptor blockers, potassium-sparing diuretics, or potassium supplements.

Taking Eplerenone with diazoxide, MAOIs, pentoxifylline, or prostacyclin analogues may increase the blood pressure-lowering effect of other antihypertensive medicines.

NSAIDs may decrease the blood pressure-lowering and diuretic effects of potassium-sparing diuretics, and can also increase the risk of high potassium levels.

There have been reports of lithium toxicity when used with ACE inhibitors or diuretics, so it is important to monitor lithium levels if used together with Eplerenone.

Contraindications

**For all patients:**
Do not use if the patient has a blood potassium level higher than 5.5 mEq/L at the start, a creatinine clearance (CrCl) of 30 mL/min or less, or is taking strong CYP3A4 inhibitors (such as ketoconazole, itraconazole, nefazodone, troleandomycin, clarithromycin, ritonavir, or nelfinavir).

**For patients being treated for high blood pressure:**
Do not use in patients with type 2 diabetes and microalbuminuria, or if serum creatinine is higher than 2 mg/dL in men or 1.8 mg/dL in women, or if CrCl is lower than 30 mL/min.

Mode of actions

Aldosterone attaches to mineralocorticoid receptors found in both epithelial tissues (like the kidneys and digestive system) and nonepithelial tissues (such as the heart, blood vessels, and brain). This causes blood pressure to rise by making the body reabsorb more sodium, hold on to more water, and change the structure of blood vessels, among other effects. Eplerenone is a drug that specifically blocks these mineralocorticoid receptors, preventing aldosterone from working and lowering blood pressure. It also seems to help protect the heart in people with congestive heart failure after a heart attack.

Dosage & Administration

Adult: PO CHF Post-MI Initial: 25 mg once daily; titrate to recommended dose of 50 mg once daily w/in 4 wk as tolerated. Adjust dose based on serum potassium level; reduce or w/draw therapy if hyperkalaemia develops.

HTN Initial: 50 mg once daily; may increase to 50 mg twice daily based on response. Initiate at lower dose of 25 mg once daily if used concurrently w/ moderate CYP3A4 inhibitors.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect even sometimes unable to demonstrate the risk on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required.
