Some people who take antipsychotics for a long time can develop tardive dyskinesia, a movement disorder.
Chlorpromazine should be used carefully in people with heart, liver, or kidney problems. People who have had liver problems from cirrhosis, especially those who have had hepatic encephalopathy, are more sensitive to the effects of chlorpromazine on the brain. This can cause confusion and slow down brain activity.
Since chlorpromazine can slow down the central nervous system, it should be used cautiously in people with long-term breathing problems like severe asthma, emphysema, or chest infections. This is especially important for children between 1 and 12 years old.
Chlorpromazine can also reduce the ability to cough, which means there is a risk of inhaling vomit into the lungs.
Using antiparkinson drugs and tricyclic antidepressants (TCAs) together can increase anticholinergic side effects and may cause a serious anticholinergic crisis. When taken with MAOIs, they can increase the risk of low blood pressure when standing up (orthostatic hypotension). They can also reduce or reverse the blood pressure-lowering effects of medications like guanethidine, methyldopa, and clonidine.
Danger: These drugs can have stronger sedative effects when combined with other sedatives, sleeping pills, antihistamines, general anesthetics, opioids, or alcohol. This combination can be life-threatening.
Do not give this medicine to people who are allergic to phenothiazines.
Do not use it if the person is in a coma or has taken a lot of substances that slow down the brain, such as alcohol, barbiturates, or narcotics.
Chlorpromazine is a type of medication that works by blocking dopamine receptors in the brain’s mesolimbic system. It also reduces the release of certain hormones from the hypothalamus and pituitary gland. In addition, chlorpromazine helps prevent nausea and vomiting (antiemetic effect), blocks some effects of serotonin, has mild antihistamine effects, and slightly affects nerve signals in ganglia.
Oral
Intractable hiccup
Adult: Initially, 25-50 mg 3-4 times daily for 2-3 days; if unresponsive, may admin 25-50 mg via IM inj. If still necessary, 25-50 mg in 500-1000 ml of normal saline may be given via slow IV infusion.
Child: 1-12 yr: 500 mcg/kg every 4-6 hr. Max: >5 yr: 75 mg daily; 1-5 yr: 40 mg daily.
Psychoses
Adult: 25 mg tid; may be given as a single 75 mg dose at night. Maintenance: 25-100 mg tid increased to ?1 g daily as required in psychotic patients.
Child: 1-12 yr: 500 mcg/kg every 4-6 hr. Max: >5 yr: 75 mg daily; 1-5 yr: 40 mg daily.

Adult: 25-50 mg repeated every 6-8 hr. Substitute with oral therapy as soon as possible.
Child: 1-12 yr: 500 mcg/kg every 4-6 hr. Max: >5 yr: 75 mg daily; 1-5 yr: 40 mg daily.

Nausea and vomiting
Adult: Initially, 25 mg via IM inj, followed by 25-50 mg every 3-4 hr until vomiting stops.
Child: 1-12 yr: 500 mcg/kg every 4-6 hr. Max: >5 yr: 75 mg daily; 1-5 yr: 40 mg daily.
Elderly: Initially, 1/3-½ the normal adult dose.
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
