Para evitar estimular el revestimiento del útero, no tome más de 8 mg por día y no use esta dosis máxima durante más de unas pocas semanas. Como ocurre con todas las terapias de reemplazo hormonal, se debe realizar un chequeo médico una vez al año. Si tienes una infección vaginal, también debes recibir el tratamiento adecuado al mismo tiempo. Los pacientes que tienen cualquiera de las siguientes condiciones deben ser monitoreados de cerca: coágulos de sangre (trombosis), problemas hepáticos graves, porfiria o antecedentes (ya sea durante el embarazo o mientras usan esteroides) de picazón intensa, ictericia relacionada con el hígado, un tipo de erupción cutánea durante el embarazo (herpes gestacional) o una afección llamada otosclerosis.
La eficacia de este medicamento puede verse reducida si se toma con carbón activado, barbitúricos, carbamazepina, griseofulvina, hidantoína o rifampicina. También puede afectar la eficacia de los anticoagulantes orales. Además, puede aumentar los efectos de la succinilcolina, las teofilinas y la troleandromicina.
- Embarazo
- Historia de cáncer de mama, o si se sospecha cáncer de mama.
- Cáncer conocido o sospechado que depende de los estrógenos, como el cáncer de endometrio (uterino).
- Sangrado vaginal inexplicable
- Engrosamiento no tratado del revestimiento uterino (hiperplasia endometrial)
- Coágulos de sangre pasados o actuales en venas profundas (como trombosis venosa profunda) o pulmones (embolia pulmonar)
- Problemas cardíacos o arteriales graves actuales o recientes, como angina o un ataque cardíaco
- Enfermedad hepática activa o antecedentes de problemas hepáticos si las pruebas hepáticas aún no son normales.
El estriol es una forma natural de estrógeno. Ayuda a restaurar el revestimiento normal de la vagina. Se cree que tiene un efecto leve sobre el crecimiento del revestimiento del útero (endometrio). El estriol permanece en los núcleos celulares del endometrio sólo por un corto tiempo.
Adultos: TRH para menopausia por vía oral 0,5-3 mg/día durante 1 mes, luego 0,5-1 mg/día.
Ayuda diagnóstica en frotis cervical atrófico: 2-4 mg/día durante 7 días antes de realizar el siguiente frotis.
Infertilidad por hostilidad cervical: 0,25-1 mg/día los días 6-15 del ciclo menstrual.
Atrofia del tracto urogenital inferior relacionada con deficiencia de estrógeno: 4-8 mg/día durante la primera semana, luego disminuir gradualmente a 1-2 mg/día.
Vaginitis atrófica menopáusica Vag: En forma de crema (0,01 o 0,1%) o pesario (500 mcg): aplique 500 mcg una vez al día, reduzca a dos veces por semana posteriormente.
Terapia pre y postoperatoria en mujeres posmenopáusicas sometidas a cirugía vaginal: 4-8 mg por día en las 2 semanas previas a la cirugía; 1-2 mg por día en las 2 semanas posteriores a la cirugía.
Molestias climatéricas como sofocos y sudoración nocturna: 4-8 mg al día durante las primeras semanas, seguido de una reducción gradual. Se debe utilizar la dosis eficaz más baja.
Este medicamento no debe usarse durante el embarazo. Puede absorberse a través de la piel y los pulmones y puede dañar los órganos del feto y causar discapacidades congénitas. Dos tipos estrictos de control de la natalidad eficaces comienzan un mes antes y durante el tratamiento y durante tres años después de suspender este medicamento. Hable con sus médicos si no está segura de qué tipos de anticonceptivos son efectivos para usted. Debe necesitar hacerse pruebas de embarazo regularmente durante su tratamiento con este medicamento y durante al menos tres años después de tomar este medicamento. Si parece que queda embarazada, deje de tomar este medicamento y llame a su médico inmediatamente. Si realmente queda embarazada, pierde un período menstrual o tiene relaciones sexuales sin usar dos métodos anticonceptivos. En algunos casos, su médico puede recetarle anticonceptivos de emergencia para prevenir el embarazo. Las personas que toman este medicamento no deben donar sangre durante el tratamiento ni durante los tres años posteriores.
Este medicamento también está contraindicado en la leche materna. No se recomienda amamantar mientras se usa este medicamento y durante los tres años posteriores.
