Precautions and warnings
गर्भाशय की परत को उत्तेजित करने से बचने के लिए, प्रति दिन 8 मिलीग्राम से अधिक न लें, और इस अधिकतम खुराक का उपयोग कुछ हफ्तों से अधिक न करें। सभी हार्मोन रिप्लेसमेंट थेरेपी की तरह, आपको साल में एक बार मेडिकल जांच करानी चाहिए। यदि आपको योनि में संक्रमण है, तो आपको उसी समय उचित उपचार भी प्राप्त करना चाहिए। जिन रोगियों में निम्नलिखित में से कोई भी स्थिति है, उन पर बारीकी से नजर रखी जानी चाहिए: रक्त के थक्के (थ्रोम्बोसिस), गंभीर यकृत की समस्याएं, पोरफाइरिया, या इतिहास - या तो गर्भावस्था के दौरान या स्टेरॉयड का उपयोग करते समय - गंभीर खुजली, यकृत से संबंधित पीलिया, एक प्रकार का गर्भावस्था दाने (हर्पस गेस्टेशनिस), या ओटोस्क्लेरोसिस नामक स्थिति।

Drug Interactions
सक्रिय चारकोल, बार्बिट्यूरेट्स, कार्बामाज़ेपाइन, ग्रिसोफुल्विन, हाइडेंटोइन या रिफैम्पिसिन के साथ लेने पर इस दवा की प्रभावशीलता कम हो सकती है। यह इस बात पर भी प्रभाव डाल सकता है कि मौखिक एंटीकोआगुलंट्स कितनी अच्छी तरह काम करते हैं। इसके अतिरिक्त, यह स्यूसिनिलकोलाइन, थियोफिलाइन और ट्रॉलिएंड्रोमाइसिन के प्रभाव को बढ़ा सकता है।
Contraindications

Mode of actions

Dosage & Administration
वयस्क: पीओ रजोनिवृत्ति एचआरटी 0.5-3 मिलीग्राम/दिन 1 महीने के लिए, फिर 0.5-1 मिलीग्राम/दिन।
एट्रोफिक सर्वाइकल स्मीयर में नैदानिक सहायता: अगला स्मीयर लेने से पहले 7 दिनों के लिए 2-4 मिलीग्राम/दिन।
गर्भाशय ग्रीवा की शत्रुता के कारण बांझपन: मासिक धर्म चक्र के 6-15वें दिन 0.25-1 मिलीग्राम/दिन।
एस्ट्रोजेन की कमी से संबंधित निचले मूत्रजनन पथ का शोष: पहले सप्ताह के लिए 4-8 मिलीग्राम/दिन, फिर कम होकर 1-2 मिलीग्राम/दिन।
वैग मेनोपॉज़ल एट्रोफिक वेजिनाइटिस: क्रीम (0.01 या 0.1%) या पेसरी (500 एमसीजी) के रूप में: प्रतिदिन एक बार 500 एमसीजी लगाएं, बाद में सप्ताह में दो बार कम करें।
रजोनिवृत्ति के बाद योनि की सर्जरी कराने वाली महिलाओं में सर्जरी से पहले और पश्चात की चिकित्सा: सर्जरी से पहले 2 सप्ताह में प्रति दिन 4-8 मिलीग्राम; सर्जरी के बाद 2 सप्ताह में प्रति दिन 1-2 मिलीग्राम।
क्लाइमेक्टेरिक शिकायतें जैसे गर्म फ्लश और रात को पसीना: पहले हफ्तों के दौरान प्रति दिन 4-8 मिलीग्राम, इसके बाद धीरे-धीरे कमी आती है। सबसे कम प्रभावी खुराक का उपयोग किया जाना चाहिए।
Pregnancy & Lactation
गर्भावस्था के दौरान इस दवा का उपयोग नहीं किया जाना चाहिए। यह त्वचा और फेफड़ों के माध्यम से अवशोषित हो सकता है, जिससे आपके अजन्मे बच्चे के अंगों को नुकसान हो सकता है और जन्मजात विकलांगता हो सकती है। जन्म नियंत्रण के सख्त दो प्रभावी प्रकार उपचार के एक महीने पहले और उसके दौरान शुरू होते हैं और इस दवा को रोकने के तीन साल बाद तक। यदि आप निश्चित नहीं हैं कि किस प्रकार का जन्म नियंत्रण आपके लिए प्रभावी है तो अपने डॉक्टरों से चर्चा करें। आपको इस दवा से उपचार के दौरान और इस दवा को लेने के बाद कम से कम तीन साल तक नियमित रूप से गर्भावस्था परीक्षण कराने की आवश्यकता होगी। यदि ऐसा लगता है कि आप गर्भवती हो गई हैं तो यह दवा लेना बंद कर दें और तुरंत अपने डॉक्टर को बुलाएँ। यदि आप वास्तव में गर्भवती हो जाती हैं, मासिक धर्म से चूक जाती हैं, या दो प्रकार के जन्म नियंत्रण का उपयोग किए बिना यौन संबंध बनाती हैं। कुछ मामलों में, आपका डॉक्टर गर्भावस्था को रोकने के लिए आपातकालीन गर्भनिरोधक लिख सकता है। इस दवा का सेवन करने वाले लोगों को इलाज के दौरान और उसके बाद तीन साल तक रक्तदान नहीं करना चाहिए।
यह दवा स्तन के दूध में भी वर्जित है। इस दवा का उपयोग करते समय और उसके बाद तीन साल तक स्तनपान की सिफारिश नहीं की जाती है।
