Precautions and warnings

Low blood volume, high potassium levels, connective tissue diseases, and narrowed heart valves can all be concerns. Problems may happen before, during, or right after anesthesia. People with existing kidney problems or those with narrowing in one of the kidney's arteries are also at risk. These issues are particularly important to consider in children.

Drug Interactions

Taking this medicine with diuretics may cause your blood pressure to drop too much. Using it together with NSAIDs can harm your kidneys and make it less effective at lowering blood pressure. It can raise the levels and side effects of lithium in your body. There’s a higher risk of high potassium levels if you also take potassium-sparing diuretics or potassium supplements. If used with gold sodium thiomalate, it might make certain side effects, like nitritoid reactions, more likely.
**Serious risks:** Combining this medication with aliskiren, especially in people with diabetes or kidney problems, can lead to dangerously low blood pressure, high potassium, and kidney issues.

Contraindications

A history of angioedema caused by previous use of ACE inhibitors, or a history of hereditary or unknown (idiopathic) angioedema. Narrowing of both kidney arteries (bilateral renal artery stenosis). Also, use during the second or third trimester of pregnancy or while breastfeeding is not recommended.

Mode of actions

Lisinopril works by blocking the enzyme ACE, which normally changes angiotensin I into angiotensin II, a chemical that narrows blood vessels. By stopping this process, lisinopril increases the activity of renin and lowers the amount of aldosterone, a hormone that causes the body to hold onto water and salt. As a result, blood vessels relax and blood pressure goes down.

Dosage & Administration

Oral
Hypertension
Adult: Initially, 10 mg/day, 1st dose given preferably at bedtime to avoid precipitous fall in BP. Patient w/ renovascular HTN, volume depletion, severe HTN: Initially, 2.5-5 mg once daily. Patient on diuretic: Initially, 5 mg once daily. Maintenance: 20 mg once daily, up to 80 mg/day may be given if needed.
Child: >6 yr Initially, 0.07 mg/kg, up to 5 mg once daily.

Renal impairment: Adult: CrCl

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
