Precautions and warnings

Discuss your medical history with your doctor before using this drug, especially if you have a history of penicillin allergy, severe renal illness, are pregnant or breastfeeding, have a superinfection, gallbladder disease, liver disease, or stomach diseases including colitis.

To a newborn, less than one month old who is receiving this drug, do not provide any calcium-containing products via a vein, including IV solutions such as Ringer's solution, Hartmann's solution, or parenteral nutrition-TPN/PPN. This can result in major issues with the key organs.

Ceftriaxone may impair the effectiveness of live bacterial vaccinations such as the typhoid vaccine. Unless your doctor advises otherwise, do not get any immunizations while using this medicine.

Drug Interactions

Calcium-containing IV fluids are one product that may interact with this medicine.

Certain lab tests, such as urine glucose testing, may be affected by this medicine, resulting in erroneous test findings. Make sure lab personnel and all of your doctors are aware that you are taking this medication.

Aminoglycosides' nephrotoxicity may be increased. BCG, typhoid vaccination, and Na picosulfate may have reduced therapeutic efficacy. Vitamin K antagonists' anticoagulant impact may be enhanced (for example, warfarin). Along with probenecid, it's possible that serum levels will rise.

Administration with a Ca-containing IV soln may result in the precipitation of a crystalline material in the lungs and kidneys, which can be fatal.

Contraindications

Neonatal hyperbilirubinemia due to hypersensitivity to the active component in cephalosporins. Because of the danger of calcium-ceftriaxone precipitation, do not use calcium or calcium-containing solutions or products within 48 hours of ceftriaxone administration.

Mode of actions

Ceftriaxone binds to one or more penicillin-binding proteins (PBPs), preventing the last transpeptidation step of peptidoglycan production in the bacterial cell wall, halting biosynthesis and causing bacterial cell death.

Dosage & Administration

Adult: IV Typhoid fever 2 g once daily for 14 days.
IV/IM Susceptible infections 1-2 g/day, up to 4 g/day for severe infections.
Surgical prophylaxis 1 g as a single dose 0.5-2 hr pre-op.
IM Uncomplicated gonorrhoea; Prevention of secondary meningococcal meningitis 250 mg as a single dose.

Children under 12 years: The recommended total daily dose is 50 to 75 mg/kg once daily (or twice daily in equally divided doses). In severe infections, up to 80 mg/kg body weight daily may be given.

The total daily dose should not exceed 2 gm. In the treatment of meningitis, the initial dose of 100 mg/kg body weight (not to exceed 4 gm daily) once daily (or twice daily in equally divided doses), is recommended.
As soon as the causative organism has been identified and its sensitivity, the doses can be reduced accordingly. The usual duration of therapy in meningitis is 7 to 14 days.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect even sometimes unable to demonstrate the risk on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required.
