Precautions and warnings

**History of airway surgery:**
Let your doctor know if you have had airway surgery before.

**Liver function:**
Stop using this medicine if your liver function tests (LFTs) increase a lot.

**Potassium:**
There is a risk of high potassium levels, so potassium should be checked often.

**Kidney function:**
If your kidneys depend on the renin-angiotensin-aldosterone system, talk to your doctor. Stop the medicine if your kidney function keeps getting worse.

**Connective tissue disease:**
If you have a disease like lupus or scleroderma, take extra care with this medicine.

**Low blood volume (hypovolaemia):**
Blood pressure may drop with the first dose, so monitor BP closely.

**Kidney and heart problems:**
This medicine may not be suitable if you have narrowing of one of your kidney arteries, kidney problems from before, or narrowing of the aortic valve in your heart.

**Anaesthesia:**
Do not use this medicine before, during, or right after having anaesthesia.

**Driving and operating machinery:**
This medicine could make you less alert, so be careful when driving or using machines.

**Breastfeeding:**
Tell your doctor if you are breastfeeding.

Drug Interactions

Taking this medicine with diuretics may increase its blood pressure-lowering effect. Using it together with potassium supplements, potassium-sparing diuretics, or other drugs like ciclosporin, heparin, or indometacin can raise your potassium levels too much. It can also increase the levels and risk of side effects of lithium in your body. The blood pressure-lowering effect may be less if you use aspirin or other NSAIDs (anti-inflammatory drugs). Using it with NSAIDs can also raise the risk of kidney problems. If you take antidiabetic drugs, there is a higher chance of low blood sugar (hypoglycaemia). Rarely, using it with gold injections (sodium aurothiomalate) can cause nitritoid reactions.

Serious Warning: In patients with diabetes or kidney problems, combining this medicine with aliskiren can be very dangerous, increasing the risk of low blood pressure, high potassium, and serious kidney problems, including kidney failure.

Contraindications

Has a history of angioedema (swelling) caused by using ACE inhibitors. Is in the 2nd or 3rd trimester of pregnancy.

Mode of actions

Perindopril is a type of medicine called an ACE inhibitor. It works by blocking the conversion of angiotensin I into angiotensin II, a substance that narrows blood vessels. By doing this, perindopril helps to relax blood vessels and lower blood pressure. It also reduces the activity of the sympathetic nervous system and blocks an enzyme called kininase, which is involved in breaking down bradykinin and other natural substances in the body.

Dosage & Administration

Adult: PO HTN As erbumine: Initial: 4 mg once daily. Patients w/ renovascular HTN, vol depletion, severe HTN: Initial: 2 mg once daily. Max: 8 mg/day.

Elderly: Initially, 2 mg (as erbumine) once daily. Dose may be increased to max 8 mg (erbumine) after 1 mth if needed.

Patients on diuretics: Withdraw diuretics 2 or 3 days before perindropil therapy. Resume later if required. If diuretic cannot be discontinued, an initial dose of 2 mg once daily. Max: 8 mg/day.

Heart failure As erbumine: Initial: 2 mg in the morning. Increase slowly if needed. Maintenance: 4 mg/day.

Stable ischaemic heart As erbumine: Initial: 4 mg once daily for 2 wk. Maintenance: 8 or 10 mg once daily.

Elderly: Initially, 2 mg mg (as erbumine) once daily on the 1st wk.

Renal impairment:
CrCl (ml/min)

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
