Precautions and warnings

Pregnancy, a woman who breastfeeds her baby, regular electrolyte monitoring (particularly potassium, calcium, and magnesium), and potassium supplements/potassium-sparing diuretics administered if hypokalemia occurs. Blood glucose, BUN, and creatinine levels, as well as blood counts, should all be monitored. Allergy to sulfonamides. Digitalis-treated CHF patients, K-losing nephropathy, hepatic cirrhosis and ascites, and diarrhea.

Drug Interactions

Probenecid has a diuretic and natriuretic effect. Indometacin blocks bumetanide's effect, hence taking it with antihypertensives raises the risk of orthostatic hypotension.

Concurrent use with ototoxic medications such as aminoglycosides and nephrotoxic drugs can be fatal. Lithium excretion through the kidneys is reduced.

Contraindications

Hypersensitivity, renal failure and anuria, hepatic coma, and severe electrolyte depletion are all symptoms of hypersensitivity.

Mode of actions

Bumetanide causes diuresis by preventing water and electrolyte (sodium and chloride) from being reabsorbed in the ascending loop of Henle and the proximal renal tubule.

Dosage & Administration

Adult: PO Oedema 1 mg once daily, 2nd dose 6-8 hr later if needed. Refractory oedema Initial: 5 mg/day, may increase dose depending on response. Max: 10 mg/day. HTN 0.5-1 mg/day. Max: 5 mg/day.

IV Pulmonary oedema 1-2 mg, repeat 20 mins later if needed. IV/IM Emergency oedema 0.5-1 mg, then adjust according to response.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
